目的:探讨骨髓间充质干细胞( MSCs)移植对重症胰腺炎( SAP)的治疗作用及相关机制。方法将雄性SD大鼠120只随机分为假手术组、模型组及移植组各40只。模型组和移植组采用5%牛磺胆酸钠逆行胰胆管注射方法制作SAP模型,假手术组开腹后翻动肠壁即关腹。造模成功后移植组经门静脉植入MSCs;模型组及假手术组采用同样方法给予等量生理盐水。于24、48、72 h每组分别处死10只大鼠,对胰腺和肾脏损伤进行大体形态和光镜下观察,测定血清淀粉酶( AMY)、BUN、Cr、TNF-α和IL-6水平。结果模型组胰腺和肾脏组织损伤明显,各时间点血清AMY、BUN、Cr、TNF-α和IL-6水平均高于假手术组,P均<0.05。移植组胰腺和肾脏组织损伤有所减轻;各时间点血清中AMY、BUN、Cr、TNF-α和IL-6水平均低于模型组及假手术组,P均<0.05。结论经门静脉移植MSCs治疗SAP效果确切,可降低多器官功能障碍的发生率。

목적:탐토골수간충질간세포( MSCs)이식대중증이선염( SAP)적치료작용급상관궤제。방법장웅성SD대서120지수궤분위가수술조、모형조급이식조각40지。모형조화이식조채용5%우광담산납역행이담관주사방법제작SAP모형,가수술조개복후번동장벽즉관복。조모성공후이식조경문정맥식입MSCs;모형조급가수술조채용동양방법급여등량생리염수。우24、48、72 h매조분별처사10지대서,대이선화신장손상진행대체형태화광경하관찰,측정혈청정분매( AMY)、BUN、Cr、TNF-α화IL-6수평。결과모형조이선화신장조직손상명현,각시간점혈청AMY、BUN、Cr、TNF-α화IL-6수평균고우가수술조,P균<0.05。이식조이선화신장조직손상유소감경;각시간점혈청중AMY、BUN、Cr、TNF-α화IL-6수평균저우모형조급가수술조,P균<0.05。결론경문정맥이식MSCs치료SAP효과학절,가강저다기관공능장애적발생솔。
Objective To study the protective effect and the possible mechanisms of transplantation bone marrow mes-enchymal stem cells (MSCs) on pancreatic injury in rats with severe acute pancreatitis(SAP).Methods A total of 120 male Sprague-Dawley rats were randomly divided into sham operation group( N group) , severe acute pancreatitis group ( P group) , transplantation MSCs group( M group) .The dose of 5%sodium taurocholate was retrograde injected into the pan-creatic duct of the rats in P group and M group to induce SAP model.Rats in N group whose pancreases were just flipped and striked gently without injection.MSCs were transplanted into rats in M group via portal vein, while the same dosage of 0.9%NS was injected by the same vein routine in P group.Ten rats in each group were sacrificed at 24 h, 48 h, 72 h lat-er.Pancreas and kidney damage of each group were observed under gross morphology and optical lens.The serum AMY, BUN, Cr, TNF-αand IL-6 level of each group were tested.Results Pancreas and kidney injury in P group were signifi-cantly heavier.The content of serum AMY, BUN, Cr, TNF-αand IL-6 level in P group were both higher than those of N group(P<0.05).Pancreatic and renal tissue injury in M group rats mitigated.The content of serum AMY, BUN, Cr, TNF-αand IL-6 level in M group were both lower than those of P group (P<0.05).Conclusions Transplantation via portal vein MSCs is an effective treatment on SAP, which can reduce the occurrence of multiple organ dysfunction and im-prove prognosis.
