目的：探讨宫腔镜联合B超在宫内节育器嵌顿中的治疗作用。方法回顾性分析2011年12月至2013年12月用宫腔镜联合B超治疗的103例宫内节育器嵌顿临床病例资料。结果期间共取环1509例，其中103例发生节育器嵌顿，各种嵌顿的节育器构成比分别为：金属圆环52例（8．11％），爱母环47例（17．21％），T型环2例（1．21％），宫型环1例（0．47％），吉妮环1例（1．37％）。节育器嵌顿发生在绝经前50例，绝经2年内10例，绝经2年以上43例。所有嵌顿节育器均在B超监测下宫腔镜下一次成功取出。结论金属圆环及爱母环较易嵌顿，绝经2年以上节育器易于发生嵌顿，应尽可能在绝经后2年内取环。对于嵌顿的宫内节育器，B超监测下宫腔镜检查及镜下取环是一种安全有效的治疗手段。
목적：탐토궁강경연합B초재궁내절육기감돈중적치료작용。방법회고성분석2011년12월지2013년12월용궁강경연합B초치료적103례궁내절육기감돈림상병례자료。결과기간공취배1509례，기중103례발생절육기감돈，각충감돈적절육기구성비분별위：금속원배52례（8．11％），애모배47례（17．21％），T형배2례（1．21％），궁형배1례（0．47％），길니배1례（1．37％）。절육기감돈발생재절경전50례，절경2년내10례，절경2년이상43례。소유감돈절육기균재B초감측하궁강경하일차성공취출。결론금속원배급애모배교역감돈，절경2년이상절육기역우발생감돈，응진가능재절경후2년내취배。대우감돈적궁내절육기，B초감측하궁강경검사급경하취배시일충안전유효적치료수단。
Objective To investigate the clinical value of hysteroscopy under ultrasound scanning in removing incarceration of IUDs. Methods Retrospective study was conducted on the clinical data of 103 patients with incarceration of IUDs treated with hysteroscopy under ultrasound scanning between December 2011 and December 2013.Results IUDs in 1 509 cases were taken out including 103 cases with incarceration of IUDs.The incarcerated IUDs included Single Steel IUDs in 52 cases (8.11%), Mother Love IUDs in 47 cases (17.21%), T copper IUDs in 2 cases (1.21%), uterine cavity type IUD in 1 case (0.47%) and Jeanne IUD in 1 case (1.37%). Fifty cases were premenopausal women.Incarceration of IUDs occurred within 2 years after menopause in 10 cases, and that occurred more than 2 years after menopause in 43 cases.All incarcerated IUDs were successfully removed with hysteroscopy under ultrasound scanning. Conclusion Single Steel IUD and Mother Love IUD are easy to incarcerate than others.IUDs are more likely to incarcerate more than 2 years after menopause.IUDs should be removed within 2 years after menopause.It is safe and effective to remove incarcerated IUDs with hysteroscopy under ultrasound scanning.
