目的：观察强化他汀治疗在症状性颅内动脉狭窄中的疗效。方法选择120例症状性颅内动脉狭窄患者，采用随机表法分为A组（阿托伐他汀10 mg/d）、B组（20 mg/d）和C组（40 mg/d）各40例，治疗至少持续12个月，随访12个月，观察临床终点事件发生率及治疗前后脑血流灌注值的变化，评价强化调脂疗效。结果3组在治疗后12个月随访时，A组、B组和C组脑血管终点事件发生率分别为26．3％、13．5％和5．4％；A组与C组比较，差异有统计学意义（ P＜0．05）。与治疗前比较，治疗后12个月，3组相对脑血流量和相对脑血容量明显升高，差异有统计学意义（P＜0.05），C组相对达峰时间明显缩短，差异有统计学意义（P＜0．05）。结论持续12个月的强化调脂治疗（40 mg/d）可以减少脑血管事件的发生，提高缺血区脑组织血流灌注。
목적：관찰강화타정치료재증상성로내동맥협착중적료효。방법선택120례증상성로내동맥협착환자，채용수궤표법분위A조（아탁벌타정10 mg/d）、B조（20 mg/d）화C조（40 mg/d）각40례，치료지소지속12개월，수방12개월，관찰림상종점사건발생솔급치료전후뇌혈류관주치적변화，평개강화조지료효。결과3조재치료후12개월수방시，A조、B조화C조뇌혈관종점사건발생솔분별위26．3％、13．5％화5．4％；A조여C조비교，차이유통계학의의（ P＜0．05）。여치료전비교，치료후12개월，3조상대뇌혈류량화상대뇌혈용량명현승고，차이유통계학의의（P＜0.05），C조상대체봉시간명현축단，차이유통계학의의（P＜0．05）。결론지속12개월적강화조지치료（40 mg/d）가이감소뇌혈관사건적발생，제고결혈구뇌조직혈류관주。
Objective To observe the effect of intensive statin therapy on symptomatic intracranial artery stenosis (ICAS) .Methods One hundred and twenty symptomatic ICAS patients were ran-domly divided into group A ,group B and group C (40 in each group) .The patients in 3 groups were treated with atorvastatin at the dose of 10 mg/d ,20 mg/d ,and 40 mg/d ,respectively ,for at least 12 months .The patients were followed up for 12 months ,during which the incidence of car-diovascular events and perfusion of ischemic brain tissues were observed .The effect of intensive statin therapy on symptomatic ICAS was assessed .Results The incidence of cardiovascular events was 26 .3% ,13 .5% and 5 .4% ,respectively ,in groups A-C after 12 months of atorvastatin treatment ,and significantly higher in group A than in group C (26 .3% vs 5 .4% ,P<0 .05) .The rCBF and rCBV were significantly higher in groups A-C after treatment than before treatment (P<0 .05) .The rTTP was shorter in group C than in groups A and B (P<0 .05) .Conclusion Twelve-month intensive statin therapy (40 mg/d) can reduce the cerebrovascular events and im-prove the perfusion of ischemic brain tissues .
