目的:探讨黑龙江地区丙型病毒性肝炎病毒基因型与亚型的分布特点及临床意义,为临床制订合理的治疗方案和判断预后提供参考依据。方法在116例抗丙型肝炎病毒(HCV)抗体和HCV RNA均为阳性的血清标本中,提取HCV RNA并进行基因分型检测。同时收集患者的基本资料、感染途径、肝功能检查结果等进行登记分析。结果黑龙江地区丙型病毒性肝炎病毒基因型以1b型为主,占50.0%,其次为2a型。不同感染途径HCV患者间基因分型构成无显著性差异,不同疾病程度患者的HCV基因型分布差异有显著性,1b型丙型病毒性肝炎患者HCV RNA含量及脂肪变性发生率均高于2a型,差异均有显著性(P均<0.05)。结论黑龙江地区流行的HCV基因型主要为1b型,并呈现一定的多样性,且HCV基因型与疾病严重程度有一定的相关性,基因型分型的检测对病情的进展及预后有一定的预测作用。

목적:탐토흑룡강지구병형병독성간염병독기인형여아형적분포특점급림상의의,위림상제정합리적치료방안화판단예후제공삼고의거。방법재116례항병형간염병독(HCV)항체화HCV RNA균위양성적혈청표본중,제취HCV RNA병진행기인분형검측。동시수집환자적기본자료、감염도경、간공능검사결과등진행등기분석。결과흑룡강지구병형병독성간염병독기인형이1b형위주,점50.0%,기차위2a형。불동감염도경HCV환자간기인분형구성무현저성차이,불동질병정도환자적HCV기인형분포차이유현저성,1b형병형병독성간염환자HCV RNA함량급지방변성발생솔균고우2a형,차이균유현저성(P균<0.05)。결론흑룡강지구류행적HCV기인형주요위1b형,병정현일정적다양성,차HCV기인형여질병엄중정도유일정적상관성,기인형분형적검측대병정적진전급예후유일정적예측작용。
Objective To explore the distribution and clinical signiifcance of genotypes and subtypes of hepatitis C virus (HCV) in Heilongjiang province and provide basis for the clinical treatment and prognosis. Method HCV RNA was extracted and genotypes and subtypes was tested in serum samples that anti HCV antibody and HCV RNA were all positive. The basic information, route of infection, the result of liver function of patients was collected at the same time. Result The main hepatitis C virus (HCV) genotype in Hei-longjiang province was 1b type, accounted for 50.0%, and followed by 2a type. The genotyping constitute between different route of infection was no signiifcant difference. The genotyping constitute between different degree of disease was statistically significant. The content of HCV RNA and fatty degeneration rate of 1b type in patients with hepatitis C were higher than 2a type (Pal <0.05). Conclusion The HCV genotype of Heilongjiang province was 1b type and had diversity, the HCV genotypes and liver disease severity has certain relevance. The detection of genotype classiifcation has prediction effect for the progress and prognosis.
