目的:探讨2型糖尿病(T2DM)患者血管内皮生长因子(VEGF)、白细胞介素-6(IL-6)和肿瘤坏死因子(TNF-α)水平的变化及其临床意义。方法应用放射免疫法对40例2型糖尿病肾病患者、36例2型糖尿病非肾病患者及38例健康人(健康对照组)进行血清VEGF、IL-6及TNF-α水平的检测。结果2型糖尿病肾病组血清VEGF、IL-6及TNF-α水平显著高于健康对照组(P<0.01)及2型糖尿病非肾病组(P<0.01)。结论 VEGF、IL-6及TNF-α是2型糖尿病肾病发生的重要炎症反应介质,提示高水平的VEGF、IL-6及TNF-α可能在T2DM及其肾病并发症的发生、发展过程中起到一定作用,检测其水平变化可作为2型糖尿病肾病临床诊断的参考依据。

목적:탐토2형당뇨병(T2DM)환자혈관내피생장인자(VEGF)、백세포개소-6(IL-6)화종류배사인자(TNF-α)수평적변화급기림상의의。방법응용방사면역법대40례2형당뇨병신병환자、36례2형당뇨병비신병환자급38례건강인(건강대조조)진행혈청VEGF、IL-6급TNF-α수평적검측。결과2형당뇨병신병조혈청VEGF、IL-6급TNF-α수평현저고우건강대조조(P<0.01)급2형당뇨병비신병조(P<0.01)。결론 VEGF、IL-6급TNF-α시2형당뇨병신병발생적중요염증반응개질,제시고수평적VEGF、IL-6급TNF-α가능재T2DM급기신병병발증적발생、발전과정중기도일정작용,검측기수평변화가작위2형당뇨병신병림상진단적삼고의거。
Objective To study the detection of the level and its clinical significance of vascular endothelial growth factor(VEGF). interleukin-6(IL-6) and tumor necrosis factor-a(TNF-α) in patients with type 2 diabetic nephropathy(DN). Methods The levels of VEGF, IL-6and TNF-α in 40 patients with DN, 36 patients diagnosed simply type 2 diabetes mellitus(DM) and 38 healthy persons (healthy controls) were radioimmunologically examined. Results The level of VEGF, IL-6 and TNF-α in the DN group were markedly lower than those in the healthy group(P < 0.01) and the simply type 2 DM group(P < 0.01). Conclusion The VEGF, IL-6 and TNF-α were important inflammatory mediators in the development of the DN, high levels of VEGF, IL-6 and TNF-α may play important roles in the occurrence and development of the type 2 diabetes mellitus and diabetic complications, detecting the level changes can be used as references for clinical diagnosis of type 2 diabetic nephropathy.
