目的:探讨老年结直肠癌组织中细胞间黏附分子1(ICAM-1)和D2-40标记的淋巴管密度(LVD)表达与临床病理因素的关系。方法选2012年3月至2013年12月普外科行手术切除的老年结直肠癌患者57例。另选正常结直肠组织20例作为对照组。免疫组化检测组织中ICAM-1表达和D2-40标记的LVD表达情况。观察ICAM-1表达和LVD与患者一般资料,肿瘤大体类型、大小、分化程度、浸润深度、淋巴结转移、TNM分期和Dukes分期的关系,并观察ICAM-1表达水平与D2-40标记的LVD之间的关系。结果结直肠癌组织中ICAM-1阳性表达率为73.7%(42/57),而正常结直肠组织中ICAM-1阳性表达率为15.0%(3/20)。结直肠癌组织中ICAM-1表达与肿瘤大小、浸润深度、淋巴结转移、TNM分期和Dukes分期相关;与患者年龄、性别、大体类型和分化程度无关。结直肠癌组织中D2-40标记LVD与肿瘤大小、分化程度、浸润深度、淋巴结转移、TNM分期和Dukes分期相关;与患者年龄、性别、大体类型无关。ICAM-1阳性表达的42例结直肠癌组织中的LVD值为(31.85±6.52),ICAM-1阴性表达组的15例结直肠癌组织中的LVD值为(25.41±4.24),两者比较差异有统计学意义( t =3.556,P <0.01)。结论 ICAM-1过表达可以上调结直肠癌的淋巴管生成从而促进结直肠癌的淋巴结转移,在结直肠癌中联合检测ICAM-1和LVD,有助于判断结直肠癌的预后及指导临床治疗。

목적:탐토노년결직장암조직중세포간점부분자1(ICAM-1)화D2-40표기적림파관밀도(LVD)표체여림상병리인소적관계。방법선2012년3월지2013년12월보외과행수술절제적노년결직장암환자57례。령선정상결직장조직20례작위대조조。면역조화검측조직중ICAM-1표체화D2-40표기적LVD표체정황。관찰ICAM-1표체화LVD여환자일반자료,종류대체류형、대소、분화정도、침윤심도、림파결전이、TNM분기화Dukes분기적관계,병관찰ICAM-1표체수평여D2-40표기적LVD지간적관계。결과결직장암조직중ICAM-1양성표체솔위73.7%(42/57),이정상결직장조직중ICAM-1양성표체솔위15.0%(3/20)。결직장암조직중ICAM-1표체여종류대소、침윤심도、림파결전이、TNM분기화Dukes분기상관;여환자년령、성별、대체류형화분화정도무관。결직장암조직중D2-40표기LVD여종류대소、분화정도、침윤심도、림파결전이、TNM분기화Dukes분기상관;여환자년령、성별、대체류형무관。ICAM-1양성표체적42례결직장암조직중적LVD치위(31.85±6.52),ICAM-1음성표체조적15례결직장암조직중적LVD치위(25.41±4.24),량자비교차이유통계학의의( t =3.556,P <0.01)。결론 ICAM-1과표체가이상조결직장암적림파관생성종이촉진결직장암적림파결전이,재결직장암중연합검측ICAM-1화LVD,유조우판단결직장암적예후급지도림상치료。
Objective Lymphatic metastasis is the prognostic factor of colorectal cancer( colorectal cancer,CRC). The aim of this re-search was to investigate the expression of ICAM-1 and D2-40 markers in colorectal cancer tissues,and the relationship between ICAM-1 and LVD expression and clinicopathological factors. We correlated the expression of ICAM-1 and LVD with general data of patients,tumor type, tumor size,differentiation degree,invasion depth,lymph node metastasis,TNM stage and Dukes stage. While we also observed the relationship between the level of ICAM-1 and LVD labeled by D2-40. Methods From Mar 2012 to December 2013,57 cases of elderly patients with colo-rectal cancer in our hospital was enrolled into this study. There were 33 males,24 females,median age was 76 years. 20 cases of normal colorectal tissues as the control group. Immunohistochemical detection of ICAM-1 and D2-40 markers for the LVD expression was performed. Results The LVD correlated with tumor size,differentiation degree,invasion depth,lymph node metastasis,TNM stage and Dukes stage in colorectal carci-noma,and has no relationship with age,gender,pathological type. In colorectal cancer,the positive rate of ICAM-1 expression was 73. 7%(42/57),while the positive expression rates of ICAM-1 in normal colorectal tissues was 15%(3/20). Expression of ICAM-1 in colorectal cancer correlated with the tumor size,depth of invasion,lymph node metastasis,TNM stage and Dukes stage;and has no relationship with age, gender,pathological type and degree of differentiation. In 42 cases of colorectal cancer tissue the positive expression of ICAM-1 in LVD value was (31. 85 ± 6. 52),ICAM-1 negative expression in 15 cases of colorectal cancer in group LVD value was(25. 41 ± 4. 24),there was statistically significant difference between them( t =3. 556,P <0. 01). Conclusion The overexpression of ICAM-1 can upregulate the lymphangiogenesis and promote lymph node metastasis in colorectal cancer. It is helpful of combined detection of ICAM-1 and LVD in the judgement of prognosis and guiding clinical treatment in colorectal cancer.
