目的:比较右美托咪定和异丙酚对乳腺癌乳房再造手术患者全身麻醉苏醒期血流动力学及镇静水平的影响,探讨右美托咪定的临床应用效果。方法:将本院2010年1月-2013年1月全麻下择期行乳腺癌乳房再造手术患者30例随机分为两组,盐酸右美托咪定组(D组)和异丙酚组(P组),每组15例。D组靶控输注右美托咪定和异丙酚,P组单纯靶控输注异丙酚。手术结束后,分别记录麻醉苏醒期患者意识清醒时(T1)、拔管时(T2)、拔管后5 min(T3)、拔管后10 min(T4)的平均动脉压(MAP)、心率(HR)和Ramsay镇静评分;记录两组患者麻醉恢复情况(苏醒时间、拔管时间和躁动例数)。结果:两组患者一般情况比较差异无统计学意义;麻醉苏醒期T1、T2时点的平均动脉压(MAP)、心率(HR),D组低于P组(P<0.05);D组术后躁动例数低于P组(P<0.05),两组苏醒时间、拔管时间和Ramsay镇静评分比较差异无统计学意义。结论:右美托咪定能减轻乳腺癌乳房再造术患者全麻苏醒期的血流动力学反应,并提供满意的镇静水平和苏醒质量。

목적:비교우미탁미정화이병분대유선암유방재조수술환자전신마취소성기혈류동역학급진정수평적영향,탐토우미탁미정적림상응용효과。방법:장본원2010년1월-2013년1월전마하택기행유선암유방재조수술환자30례수궤분위량조,염산우미탁미정조(D조)화이병분조(P조),매조15례。D조파공수주우미탁미정화이병분,P조단순파공수주이병분。수술결속후,분별기록마취소성기환자의식청성시(T1)、발관시(T2)、발관후5 min(T3)、발관후10 min(T4)적평균동맥압(MAP)、심솔(HR)화Ramsay진정평분;기록량조환자마취회복정황(소성시간、발관시간화조동례수)。결과:량조환자일반정황비교차이무통계학의의;마취소성기T1、T2시점적평균동맥압(MAP)、심솔(HR),D조저우P조(P<0.05);D조술후조동례수저우P조(P<0.05),량조소성시간、발관시간화Ramsay진정평분비교차이무통계학의의。결론:우미탁미정능감경유선암유방재조술환자전마소성기적혈류동역학반응,병제공만의적진정수평화소성질량。
Objective:To compare influence of Dexmedetomidine and Propofol on hemodynamics and the level of sedation in general anesthesia awakening period of breast cancer breast reconstruction surgery,to explore the clinical application effect of Dexmedetomidine. Method:From January 2010 to January 2013 ,30 cases of breast cancer patients with breast reconstruction surgery under general anesthesia were randomly divided into two groups,Dexmedetomidine group(D group)and Propofol group(P group),15 cases in each group. Group D target controlled infusion Dexmedetomidine,P group of target controlled infusion of propofol alone. After the surgery,mean arterial pressure(MAP), heart rate(HR)and Ramsay sedation scores of two groups were recorded in the conscious(T1),extubation(T2), 5 min after extubation(T3),10 min after extubation(T4);and anesthesia recovery(waking time,extubation time and agitation cases)of two groups were recorded.Result:The general situation was no statistical difference between the groups.The mean arterial pressure(MAP),heart rate(HR)of each anesthesia point in D group were lower than those of P group (P<0.05). Postoperative agitation cases of D group was lower than that of P group(P<0.05),awakening time,extubation time and Ramsay sedation scores of two groups was no statistical difference.Conclusion:Dexmedetomidine can reduce hemodynamic response in general anesthesia awakening period of breast cancer breast reconstruction surgery,and provide satisfactory sedation level and quality of awakening.
