目的:探究超声引导下乳腺穿刺活检术对乳腺微小肿块的临床诊疗价值。方法随机选取2009年10月~2013年10月湖南省岳阳市一人民医院乳腺微小肿块患者60例,对其行超声引导下乳腺穿刺活检术。记录超声成像和病理诊断结果。结果超声诊断结果有15例良性和41例恶性,病理诊断结果有16例良性和43例恶性。二者比较差异无统计学意义(P>0.05)。对比观察结果可见,乳腺癌患者的病灶形态不完整,边缘光滑程度差,回声多不均匀,后方回声衰减程度大,病灶钙化严重,纵横比明显偏高,超声声像图病灶形态特征与乳腺癌微小肿块比较,血流速度峰值(SPVmax)及阻力指数(RI)较高,且血流分级以Ⅱ、Ⅲ级为主。结论超声引导下穿刺活检技术可以有效诊断和治疗乳腺微小肿块。超声可有效确定病变区域和性质。穿刺活检术可有效取样并利于治疗。因此,临床上可推广使用此方法治疗乳腺肿块患者。

Objective To research the clinical diagnosis value of applying breast biopsy guided by ultrasound diagnos-ing in patients with small breast lump. Methods Sixty cases of patients with small breast lump in the First People's Hospital of Yueyang City from October 2009 to October 2013 were selected randomly. They were diagnosed by breast biopsy guided by ultrasound. The results of ultrasonic image and pathological diagnosis were recorded. Results There were 15 cases of benign and 41 cases of malignant according to the ultrasonic image. There were 16 cases of benign and 43 cases of malignant according to the result of pathological diagnosis. There was no obvious difference between the two groups (P> 0.05). The ultrasound morphological characteristics of patients with breast cancer were significantly different from the patients with proliferation of mammary gland, the values of SPVmax and RI were higher, and there was a predominance ofII andⅢ grades patients in grade of blood flow. Conclusion Breast biopsy guided by ultrasound is well in diagnosing patients with small breast lump. Ultrasound can determine the exact position and character of lesion area. Breast biopsy can sampling effectively and is beneficial to treating. Therefore, this method can be popularizing in clinic to treat breast lump patients.
