目的:对大肠癌和大肠息肉中X射线交叉互补修复基因2(X-ray repair cross-complementing gene 2,XRCC 2)和X射线交叉互补修复基因3(XRCC 3)所编码的蛋白检测,探讨二者在大肠癌及大肠息肉中的作用,评价其作为早期诊断和预后判断的生物学标志的可能性。方法选取手术切除确诊为大肠癌并未做放疗、化疗的标本218例;选取电子结肠镜下采集、病理证实为大肠腺瘤性息肉的标本33例,另取36例基本正常的大肠黏膜标本作为对照,通过免疫组化的方法,观察XRCC 2、XRCC 3所编码蛋白的表达。结果(1)XRCC 2蛋白、XRCC 3蛋白在大肠癌的表达分别为80.28%(175/218)、74.77%(163/218),显著高于息肉组的表达率57.58%(19/33)、54.55%(18/33)和正常黏膜组27.78%(10/36)、25%(9/36)(P<0.05),息肉组蛋白表达显著高于正常黏膜组,差异有统计学意义(P<0.05),在直肠癌的表达率分别为81.74%(94/115)、79.13%(91/115),左半结肠癌的表达率分别为92.19%(59/62)、79.03%(49/62),均高于息肉组,右半结肠癌的表达率分别为62.86%(22/41)、56.10%(23/41),与息肉组差异无统计学意义;(2)癌组织中XRCC 2和XRCC 3蛋白的表达呈明显正相关关系(P<0.05)。结论 XRCC 2及XRCC 3蛋白表达在大肠癌、大肠腺瘤性息肉中上调,在不同的大肠癌部位有不同的影响,同时二者可能具有协同作用,因此联合检测XRCC 2、XRCC 3有助于诊断和预警大肠癌。

목적:대대장암화대장식육중X사선교차호보수복기인2(X-ray repair cross-complementing gene 2,XRCC 2)화X사선교차호보수복기인3(XRCC 3)소편마적단백검측,탐토이자재대장암급대장식육중적작용,평개기작위조기진단화예후판단적생물학표지적가능성。방법선취수술절제학진위대장암병미주방료、화료적표본218례;선취전자결장경하채집、병리증실위대장선류성식육적표본33례,령취36례기본정상적대장점막표본작위대조,통과면역조화적방법,관찰XRCC 2、XRCC 3소편마단백적표체。결과(1)XRCC 2단백、XRCC 3단백재대장암적표체분별위80.28%(175/218)、74.77%(163/218),현저고우식육조적표체솔57.58%(19/33)、54.55%(18/33)화정상점막조27.78%(10/36)、25%(9/36)(P<0.05),식육조단백표체현저고우정상점막조,차이유통계학의의(P<0.05),재직장암적표체솔분별위81.74%(94/115)、79.13%(91/115),좌반결장암적표체솔분별위92.19%(59/62)、79.03%(49/62),균고우식육조,우반결장암적표체솔분별위62.86%(22/41)、56.10%(23/41),여식육조차이무통계학의의;(2)암조직중XRCC 2화XRCC 3단백적표체정명현정상관관계(P<0.05)。결론 XRCC 2급XRCC 3단백표체재대장암、대장선류성식육중상조,재불동적대장암부위유불동적영향,동시이자가능구유협동작용,인차연합검측XRCC 2、XRCC 3유조우진단화예경대장암。
Objective To explore the role in colorectal cancinoma and polypus by detecting the protein expression of XRCC 2, XRCC 3 , and evaluate its possibility as a biological marker for early diagnosis and prognosis. Methods Colorectal carcinoma with clear pathological diagnosis from 218 patients were collected. Immunohistochemistry was used to detect the protein expressions of XRCC 2 and XRCC 3 in carcinoma group of all the patients and in polypus group of 33 patients, normal group of 36. Results 1 the protein expression of XRCC 2, XRCC 3 of colorectal carcinoma was significantly higher than polypus group and normal group (P<0.05), the polypus group significantly higher than the normal group (P<0.05), colorectal carcinoma and the left colon carcinoma significant than polypus, there were no significant difference between right colon carcinoma and polypus;2 The protein expression of XRCC 2 and XRCC 3 was positively correlated in carcinoma (P<0.05). Conclusion The expression levels of XRCC 2 and XRCC 3 in colorectal carcinoma, polypus were up-regulated, have different effects in different parts of the colorectal carcinoma , and may have a synergistic effect, therefore it may be helpful to detect the protein expressions of both XRCC 2 and XRCC 3 for the diagnosis and predication of colorectal carcinoma.
