目的:通过研究阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者经持续气道正压通气(CPAP)治疗前后外周血中IL-7和调节性T (T reg )细胞的水平变化,探讨IL-7和T reg细胞在OSA HS患者中病理生理机制。方法采集OSA HS患者(OS-A HS组,51例)治疗前以及健康体检者(对照组,35例)人群外周静脉血标本,分别采用ELISA及流式细胞术(FCM )检测受试者外周血中IL-7和Treg细胞水平,并分析OSAHS患者外周血IL-7和Treg细胞水平与睡眠呼吸暂停低通气指数(AHI)和最低血氧饱和度(SaO2)的相关性;OSAHS组患者经CPAP治疗后再次检测上述各项指标,比较其治疗前后的水平变化。结果 OS-AHS组患者治疗后外周血IL-7表达[(186.41±18.10)pg/mL]较治疗前明显增高[(87.39±5.16)pg/mL],Treg细胞数量在治疗后为(1.28±0.19)%,较治疗前[(0.95±0.11)%]明显升高。相关性分析显示,OSAHS患者外周血IL-7及 Treg细胞水平与最低SaO2呈正相关(r=0.679,r=0.681,P<0.01);OSAHS患者外周血IL-7及 Treg细胞水平与 AHI呈负相关(r=-0.653, r=-0.632,P<0.01)。结论 OSAHS患者外周血中IL-7和Treg细胞水平较对照组降低可能是OSAHS患者高水平系统炎症的原因之一,CPAP治疗能有效提高OSAHS患者的IL-7与Treg细胞水平,改善患者机体免疫失衡状态。

Objective To explore the changes of IL-7 and Treg cells levels in peripheral blood ,and the effect of continuous posi-tive airway pressure(CPAP)therapy in patients with obstructive sleep apnea hypopnea syndrome(OSAHS) .Methods Peripheral blood levels of IL-7 and Treg cells were detected in 51 OSAHS patients before and after CPAP therapy as well as in 35 normal con-trols by using ELISA and flow cytometry(FCM) respectively .The relationship between IL-7 ,Treg cells and apnea hypopnea index (AHI) ,Oxygen Saturation(SaO2 )were analysised .Results The levels of IL-7 and Treg cells in OSAHS disorders were(87 .39 ± 5 .16)pg/mL and(0 .95 ± 0 .11)% (P<0 .01) .Notably ,the expression of IL-7 and Treg cells in those patients greatly increased after treatment with CPAP for 3 months [(186 .41 ± 18 .10)pg/mL ,(1 .28 ± 0 .19)% ,P<0 .01)] .Moreover ,the levels of IL-7 and Treg cells were significantly positively correlated with SaO2 (r=0 .679 ,r=0 .681 ,P<0 .01)but were meaningfully negatively correlated with the AHI(r= -0 .653 ,r= -0 .632 ,P<0 .01) .Conclusion The expression of IL-7 and Treg cells declined in OSAHS pa-tients ,which may produce heightened levels of inflammation in those disorders .Moreover ,the CPAP therapy could effectively in-crease the expression of IL-7 and Treg cells in patients with OSAHS .
