目的:探讨高值甲胎蛋白出现钩状效应的情况以及钩状效应对实验结果产生的影响。方法取其中79例原发性肝癌患者的高值甲胎蛋白(>105μg/L)标本进行倍比稀释,用电化学发光法(罗氏)和化学发光法(索灵)检测并观察是否会出现钩状效应,并对其结果进行比较分析。结果通过两种方法检测发现在甲胎蛋白浓度超过4×104μg/L 的标本中有可能出现钩状效应,且出现钩状效应的点均在最大检测范围之上(罗氏的有效线性范围0~1210μg/L;索灵有效线性范围0~1000μg/L)。同时,用两种方法验证在线性范围内的结果线性良好。结论电化学发光法和化学发光法检测高值甲胎蛋白均存在钩状效应,并且因出现钩状效应的点在最大检测范围之上,从而较好的避免了因钩状效应而导致异常低值结果。建议在用这两种方法检测甲胎蛋白时,对于出现超过有效线性范围的结果进行有效稀释;而对于线性范围内的结果均可视为试验的真实结果。

Objective To discuss the hook effect induced by high value Alpha fetoprotein(AFP)and its influ-ence for experimental results.Methods Seventy nine cases of high value AFP(> 105 μg/L)specimens of patients with primary liver cancer were double diluted with electrochemical luminescence method and chemiluminescence method respectively,then observed whether the hook effect take place,and the experimental results were compared and analyzed.Results The detecting results with two methods showed that the concentrations exceeded 4 ×104 μg/L yield hook effect and the barb points were above the maximum detection range(Roche Combas e601:0-1 210 μg/L;LIASON XL-100:0-1 000 μg/L).Meantime,the results in the linear range with two methods varied linearly with the effective signal values.Conclusion The hook effect was observed in the results of both methods.Moreover,the e-mergence of abnormally low value caused by the hook effect was avoided as the barb points were above the maximum detection range.It′s suggested that the specimens should be effectively diluted for the results above the maximum tes-ting range when both of the above methods are employed.And the results within the linear range could be regarded as the real results.
