目的:探讨血清特异性 IgE(sIgE)检测在婴幼儿湿疹及儿童特应性皮炎诊断中的临床意义。方法采用欧蒙印迹法检测707例婴幼儿湿疹(647例)及儿童特应性皮炎(60例)患儿血清中吸入性和食物过敏原 sIgE。结果本研究样本中引起血清 sIgE 升高的吸入性过敏原主要为尘螨组合和屋尘,食物过敏原主要为鸡蛋白。不同年龄段患儿对应的主要致敏原不同,3岁以内者鸡蛋白 sIgE 阳性率高于其他年龄组,3~6岁以及6岁以上者尘螨和屋尘 sIgE 阳性率高于3岁以下,差异均具有统计学意义。合并鼻炎或哮喘的特应性皮炎患儿与单纯婴幼儿湿疹患儿相比,屋尘过敏更多见(P =0.004)。结论尘螨组合、屋尘和鸡蛋白等吸入性和食物性过敏原是引起婴幼儿湿疹及儿童特应性皮炎的重要原因。早期采取有效环境控制,对于预防婴幼儿湿疹及儿童特应性皮炎的发生非常重要。

Objective To investigate the clinical application of serum specific IgE in children with eczema or atopic dermatitis(AD).Methods The total IgE and specific IgE for aero-and food-allergens were determined by line immunoassay(EUROLINE system,EUROIMMUN)in 707 children with eczema or AD.Results The primary com-mon aeor-allergens were dust mite and house dust;the most common food-allergen was egg white.Children in differ-ent age groups were found allergic to different allergens.The positive rate of sIgE to egg white was significantly high-er in children under 3-year old than the other age groups,and the positive rates of sIgE to dust mite and house dust significantly increased with age.Compared to children only with eczema,children with AD and accompanying rhinitis or asthma were allergic to house dust more commonly(P =0.004).Conclusion It′s suggested that aero-allergens and food-allergens such as dust mite,house dust and egg white are important causes to induce eczema and AD in children. The early effective improvement of the environment will be necessary for the management and preventions of AD in children.
