目的:评价血清降钙素原(PCT)浓度在败血症早期诊断中的临床价值。方法搜集2013年被诊断为败血症的患者327例(患者个体间无亲缘关系),对其检测的 PCT 值和血液培养结果采用 Mann-Whitney U 检验以及 ROC 曲线进行统计分析。结果(1)PCT 在血培养阳性、阴性组之间差异有统计学意义(U=6165.500,P <0.05);(2)ROC 曲线下面积为0.811,95%可信区间为0.759~0.863,标准误为0.027,可诊断为败血症的 PCT 最佳值为0.507 ng/mL,此时约登指数最大,灵敏度为0.801,特异度为0.314。结论PCT 检测值用于判断败血阴性和阳性有意义,且 PCT 检测值越大预示血培养阳性的可能性越大,因此,PCT 浓度监测可快速地明确患者是否发生细菌感染,为进一步治疗提供参考依据。如果单纯用 PCT 检测值诊断败血症,其特异度较低,尚需联合其他诊断方法。

Objective To evaluate the clinical value of serum PCT concentrations in the early diagnosis of sep-sis.Methods 327 unrelated patients diagnosed with sepsis were collected in 2013.Then the results of PCT and the blood culture about these patients were analyzed by Mann-Whitney U test and ROC curve.Results (1)The values of PCT between positive and negative blood culture groups had significant difference(U=6 165.500,P <0.05).(2)The area under the ROC curve was 0.811,95% confidence interval for the area was 0.759 to 0.863,the standard error of the area was 0.027.3.The best diagnostic value of PCT for sepsis was 0.507 ng/mL with Youden index in maxi-mum,the sensitivity as 0.801 and the specificity as 0.314.Conclusion The values of PCT between positive and nega-tive sepsis groups have significant difference(P <0.05).The blood culture was more possible positive when the level of PCT was higher.Monitoring PCT for patients with suspected bacterial infection can quickly make a definite diag-nosis,and provide reference for further treatment.
