目的:研究Laceback联合Ⅱ类牵引(56.6克力)在直丝弓排齐整平阶段对上、下颌切牙、尖牙和第一磨牙在前后向和垂直向的影响。方法选择32例安氏Ⅰ类或轻度安氏Ⅱ类1分类错牙合的正畸患者,需要拔除四颗第一双尖牙,使用 MBT 直丝弓矫治器矫治。在排齐阶段通过Laceback联合Ⅱ类牵引促进上尖牙的远中移动,下磨牙的近中移动。在矫治前和直到更换0.019英寸×0.025英寸镍钛丝整平时拍摄头颅侧位定位片和取记存模型,对测量结果进行统计分析。结果上切牙和上尖牙分别远中移动1.45 mm和3.61 mm,下切牙和下尖牙分别远中舌向移动0.27 mm和0.89 mm,而下切牙舌向移动无统计学意义。上磨牙和下磨牙分别近中移动0.51 mm和2.24 mm,上切牙和下磨牙分别伸长0.24 mm和1.03 mm,有统计学意义。OP-FH角和SN-MP角增大0.85°和0.49°,但无统计学意义。结论 Laceback联合56.6 g的Ⅱ类牵引力在直丝弓排齐阶段能有效地远中移动上颌尖牙和近中移动下颌磨牙,而下尖牙和上磨牙移动较少,利于尖牙和磨牙中性关系的维持或建立,对牙合平面和下颌平面无明显影响。

Objective To investigate the effects of Laceback ligatures combined classⅡ(56.6 g) on the anteroposterior and vertical posterior of upper and lower canines and incisors and the first molars. Methods Thirty-two subjects were included in this study who had an Angle ClassⅠ or mild ClassⅡmolar relationship. MBT brackets were used in all patients with the extraction of first premolars to solve crowding in the upper and lower arches. Laceback combined ClassⅡelastics were applied for upper canine distalization and lower molar mesialization. Lateral cephalometric radiographs and models were taken before treatment and when the working 0.019×0.025-inch NiTi archwire was placed. Results The upper incisors and canines distal movement was 1.45 mm and 3.61 mm and the lower incisors and canines distal movement was 0.27 mm and 0.89 mm respectively while lower incisors were not significant. Mesial movement of upper and lower the first molars was 0.51 mm and 2.24 mm respectively. Upper incisors and lower the first molars extrude by 0.24 mm and 1.03 mm which had statistical significance. OP-FH and SN-MP were increased by 0.85 and 0.49 degree respectively but the changes were not statistically significant. Conclusion Laceback combined Class Ⅱ elastics (56.6 g) proved to be effective for upper canine distalization during aligning stage while less lower canine and upper molar movement is found. This will help to maintain or establish canines and molars neutral relationship and no adverse effects on mandible and occlusion plane can be found.
