目的:探讨任务导向性训练对急性和亚急性期脑卒中患者步行能力的影响。方法:将91位病程少于1年的脑卒中患者随机分为以任务导向性训练的康复组(46例)和常规训练的康复对照组(45例),进行每次60分钟,每周3次,共6周的治疗。治疗前后均采用6m步行试验(6M-WT)、舒适的步行速度( CWS)、最大步行速度( MWS)、起立-行走计时测试( TUGT)、Berg平衡量表分数( BBS)评定2组患者的疗效。结果:治疗6周后,2组6M-WT、CWS、MWS、BBS与治疗前比较均有明显提高, TUG所用时间明显降低(均P<0.05);2组间比较,第2组各项指标改变值明显高于第1组(均P<0.05)。结论:以任务为导向的步行训练对急性期和亚急性期的脑卒中患者更有康复疗效。

목적:탐토임무도향성훈련대급성화아급성기뇌졸중환자보행능력적영향。방법:장91위병정소우1년적뇌졸중환자수궤분위이임무도향성훈련적강복조(46례)화상규훈련적강복대조조(45례),진행매차60분종,매주3차,공6주적치료。치료전후균채용6m보행시험(6M-WT)、서괄적보행속도( CWS)、최대보행속도( MWS)、기립-행주계시측시( TUGT)、Berg평형량표분수( BBS)평정2조환자적료효。결과:치료6주후,2조6M-WT、CWS、MWS、BBS여치료전비교균유명현제고, TUG소용시간명현강저(균P<0.05);2조간비교,제2조각항지표개변치명현고우제1조(균P<0.05)。결론:이임무위도향적보행훈련대급성기화아급성기적뇌졸중환자경유강복료효。
Objective:To investigate the effect of task oriented training on walking ability of acute and subacute stroke patients .Mteh-ods:91 patients with stroke were randomly divided into course less than a year to task oriented training of rehabilitation group (46 cases) and routine rehabilitation training group (45 cases) , for 60 minutes each time, 3 times a week, for 6 weeks.Before and after treatment with 6 meters walking test (6M-WT), comfortable walking speed (CWS), maximum walking speed (MWS), the timed up and go test (TUGT), Berg balance scale (BBS) score to assess the curative effect of two groups of patients.Results:After 6weeks treatment , 2 groups of6M-WT, CWS, MWS, BBS compared with before treatment were significantly improved with time , TUG decreased significantly (P<0.05);the comparison between 2 groups, second groups of indexes were significantly higher than that of group first (P<0.05). Conclusi on:The“training task oriented” walking on more rehabilitation of stroke patients in acute and subacute stage.
