目的：观察中间入路甲状腺全切除术的临床效果。方法回顾性分析2013年1月~2014年1月本院收治的120例甲状腺全切除术患者的临床资料，将患者随机分为观察组与对照组，各60例。观察组采用中间入路甲状腺全切除术，对照组采用传统甲状腺全切除术，对比两组的手术时间、术中出血量、术后甲状旁腺功能低下及喉返神经损伤发生率。结果观察组手术时间短于对照组，术中出血量少于对照组，术后甲状旁腺功能低下、喉返神经损伤发生率低于对照组，差异有统计学意义(P<0.05)。两组均未出现永久性甲状旁腺功能低下与喉返神经损伤的病例。结论同传统甲状腺全切除术相比，中间入路甲状腺全切除术更加安全、可靠，值得临床推广应用。
목적：관찰중간입로갑상선전절제술적림상효과。방법회고성분석2013년1월~2014년1월본원수치적120례갑상선전절제술환자적림상자료，장환자수궤분위관찰조여대조조，각60례。관찰조채용중간입로갑상선전절제술，대조조채용전통갑상선전절제술，대비량조적수술시간、술중출혈량、술후갑상방선공능저하급후반신경손상발생솔。결과관찰조수술시간단우대조조，술중출혈량소우대조조，술후갑상방선공능저하、후반신경손상발생솔저우대조조，차이유통계학의의(P<0.05)。량조균미출현영구성갑상방선공능저하여후반신경손상적병례。결론동전통갑상선전절제술상비，중간입로갑상선전절제술경가안전、가고，치득림상추엄응용。
Objective To observe the clinical effect of intermediate approach of total thyroidectomy. Methods Clinical data of 120 cases with thyroid total thyroidectomy from January 2013 to January 2014 in our hospital were retrospec-tively analysed.All patients were randomly divided into observation group and control group,60 cases in each group.The intermediate approach total thyroidectomy was used in observation group,the traditional thyroidectomy was applied in control group.The operation time,intraoperative bleeding,postoperative incidence rate of hypoparathyroidism and recur-rent laryngeal nerve injury in two groups was compared respectively. Results The operation time in observation group was shorter than that in control group,intraoperative bleeding in observation group was less than that in control group, incidence rate of hypoparathyroidism and recurrent laryngeal nerve injury in observation group was lower than that in control group respectively,with statistical difference(P<0.05).There were no permanent hypoparathyroidism and recurrent laryngeal nerve injury in two groups. Conclusion Compared with traditional thyroidectomy,intermediate approach total thyroidectomy is safe and reliable,is worthy of popularization and application in clinic.
