目的：分析玻璃体切除手术治疗高度近视患者视网膜脱离的临床特点。方法2011年4月至2012年11月，48例（48只眼）视网膜脱离合并高度近视患者，接受经平坦部玻璃体切除手术治疗，50岁以上患者均同时行晶状体（或白内障）超声乳化摘除联合人工晶状体植入，玻璃体腔内填充硅油。术后4～6个月左右行硅油取出手术。随访10个月，观察视力、眼压、视网膜复位等情况。结果22只眼视力不同程度提高，19只眼视力维持原状，7只眼视力下降。视网膜复位良好，未见视网膜再次脱离。无增生性玻璃体视网膜病变和继发性青光眼等情况出现。结论在玻璃体切除手术治疗高度近视视网膜脱离时，需综合考虑高度近视眼球的多部位病理变化特点，在术中适当改变手术技巧，并联合硅油填充，可明显促进视网膜复位，提高手术成功率。
목적：분석파리체절제수술치료고도근시환자시망막탈리적림상특점。방법2011년4월지2012년11월，48례（48지안）시망막탈리합병고도근시환자，접수경평탄부파리체절제수술치료，50세이상환자균동시행정상체（혹백내장）초성유화적제연합인공정상체식입，파리체강내전충규유。술후4～6개월좌우행규유취출수술。수방10개월，관찰시력、안압、시망막복위등정황。결과22지안시력불동정도제고，19지안시력유지원상，7지안시력하강。시망막복위량호，미견시망막재차탈리。무증생성파리체시망막병변화계발성청광안등정황출현。결론재파리체절제수술치료고도근시시망막탈리시，수종합고필고도근시안구적다부위병리변화특점，재술중괄당개변수술기교，병연합규유전충，가명현촉진시망막복위，제고수술성공솔。
Objective To study the clinical features of retinal detachment in high myopia patients .Methods Vitrectomy was performed in 48 patients ( 48 high myopic eyes ) to treatretinal detachment from April 2011 to November 2012 in our department .Patients older than 50 years received a concurrent phacoemulsification and intraocular lens implan -tation.Vitreous cavity was filled with silicone oil , which was removed about 4-~6 months after the surgery .The patients were followed-up for 10 months.Visual acuity, intraocular pressure, and retinaconditions were examined .Results Visual acuity improved in 22eyes, and remained unchanged in 19 eyes.There wasno proliferative vitreous retinopathy or secondary glaucoma reported .Conclusion Pathological changes in the high myopic eyes should be taken into considerations when we treat retinal detachment in these eyes .Appropriate choices in surgical technique and filling with silicone oil may significant-ly improvethe outcomes .
