目的：探讨前臂非常规自体动静脉瘘在维持性血液透析患者中的临床应用和护理方法。方法2007年12月至2013年12月对119例维持性透析患者进行前臂非常规自体动静脉瘘手术，其中，81例为原桡动脉-头静脉内瘘失功，38例为初次行血管内瘘手术。所有患者均有针对性地采用系统化护理干预。结果在119例患者中，109例患者手术一次取得成功（92％），术后血流量200～350 ml/min。对患者的心功能未造成不良影响，未出现严重并发症。结论有针对性的系统化护理干预能够有效提高前臂非常规自体动静脉手术的成功、成熟和使用。
목적：탐토전비비상규자체동정맥루재유지성혈액투석환자중적림상응용화호리방법。방법2007년12월지2013년12월대119례유지성투석환자진행전비비상규자체동정맥루수술，기중，81례위원뇨동맥-두정맥내루실공，38례위초차행혈관내루수술。소유환자균유침대성지채용계통화호리간예。결과재119례환자중，109례환자수술일차취득성공（92％），술후혈류량200～350 ml/min。대환자적심공능미조성불량영향，미출현엄중병발증。결론유침대성적계통화호리간예능구유효제고전비비상규자체동정맥수술적성공、성숙화사용。
Objective To investigate the clinical application and nursing of surgically created the unconventional way arteriovenous fistulas for hemodialysis access .Methods Consecutive patients in whom the forearm unconventional way of arteriovenous fistulas was conducted from December 2007 to December 2013 were reviewed .All of the patients obtained systematic nursing intervention .Results Arteriovenous fistulas creation was successful in 109 patients (92% ) .The post-operative blood flow was 200-350 ml/min . No side effect was found and cardiac function was well preserved . Conclusions Systematic nursing intervention could effectively improve success rate , mature and services of the unconventional way arteriovenous fistulas for hemodialysis access .
