目的：以1例带状疱疹继发性血小板减少症患者的药学监护过程为例，展现临床药学监护的工作方法，探讨药学监护在保证合理用药方面的价值。方法：临床药师通过科室查房、临床药学会诊等方式发现监护目标，全程参与药物治疗过程，分析药物治疗方案，协助调整和优化用药方案。结果：继发性血小板减少症与不合理使用抗病毒药相关，药师及时发布院内不合理用药警示：更昔洛韦不得用于带状疱疹的治疗。结论：临床药师开展药学监护有助于及时发现不合理用药导致的药物损伤，减少药物引发的不良事件，保证患者用药安全、有效、经济。
목적：이1례대상포진계발성혈소판감소증환자적약학감호과정위례，전현림상약학감호적공작방법，탐토약학감호재보증합리용약방면적개치。방법：림상약사통과과실사방、림상약학회진등방식발현감호목표，전정삼여약물치료과정，분석약물치료방안，협조조정화우화용약방안。결과：계발성혈소판감소증여불합리사용항병독약상관，약사급시발포원내불합리용약경시：경석락위불득용우대상포진적치료。결론：림상약사개전약학감호유조우급시발현불합리용약도치적약물손상，감소약물인발적불량사건，보증환자용약안전、유효、경제。
OBJECTIVE:To demonstrate the approach to clinical pharmaceutical care with the pharmaceutical care for one patient with herpes zoster and secondary thrombocytopenia as example, and discuss the role of the pharmaceutical care in the rational use of drug. METHODS: By performing ward round and clinical pharmaceutical consultation, pharmacist found the object of pharmaceutical care, participated in the whole treatment process of the patient, analyzed medication protocol, and assisted physicians in adjusting and optimizing medication regimen. RESULTS:The secondary thrombocytopenia is associated with irrational use of antiviral drugs. The pharmacist gave warnings on irrational use of drug in hospital by emphasizing that ganciclovir can not be used for herpes zoster. CONCULSIONS:The pharmaceutical care carried out by clinical pharmacists can help detect the drug-induced injury caused by irrational use of drugs, reduce the incidence of adverse drug events and ensure patients’ safe, effective and economic use of drugs.
