目的:观察 X线引导下大鼠退变髓核腰交感干注射对腰交感神经干炎症因子表达的影响。方法实验用雄性 SD大鼠共72只,随机分为三组:空白对照组、假手术组、髓核注射组。髓核注射组大鼠行 X线下自体退变髓核悬液右侧腰交感干注射,假手术组注射等剂量生理盐水,空白对照组不做任何处理。注射后14天,取各组大鼠右侧腰交感干行组织学观察,并采用 ELISA法定量分析炎性细胞因子 IL-1β、IL-6和 TNF-α表达变化。结果髓核注射组大鼠腰交感干组织学观察发现有明显的炎性细胞浸润及血管肿胀、充血;ELISA检测发现腰交感干 IL-1β(20.1±4.40 pg/ml),IL-6(26.5±8.9 pg/ml)及 TNF-α(60.77±6.37 pg/ml)蛋白含量比空白对照组 IL-1β(8.02±1.31 pg/ml),IL-6(10.30±1.43 pg/ml),TNF-α(24.38±4.81 pg/ml)及假手术组 IL-1β(7.52±2.11 pg/ml),IL-6(11.58±2.56 pg/ml),TNF-α(26.12±6.81 pg/ml)表达显著增高(P<0.05)。结论大鼠自体退变髓核导致腰交感神经干炎症因子表达升高可能与盘源性内脏痛的发生有关。

목적:관찰 X선인도하대서퇴변수핵요교감간주사대요교감신경간염증인자표체적영향。방법실험용웅성 SD대서공72지,수궤분위삼조:공백대조조、가수술조、수핵주사조。수핵주사조대서행 X선하자체퇴변수핵현액우측요교감간주사,가수술조주사등제량생리염수,공백대조조불주임하처리。주사후14천,취각조대서우측요교감간행조직학관찰,병채용 ELISA법정량분석염성세포인자 IL-1β、IL-6화 TNF-α표체변화。결과수핵주사조대서요교감간조직학관찰발현유명현적염성세포침윤급혈관종창、충혈;ELISA검측발현요교감간 IL-1β(20.1±4.40 pg/ml),IL-6(26.5±8.9 pg/ml)급 TNF-α(60.77±6.37 pg/ml)단백함량비공백대조조 IL-1β(8.02±1.31 pg/ml),IL-6(10.30±1.43 pg/ml),TNF-α(24.38±4.81 pg/ml)급가수술조 IL-1β(7.52±2.11 pg/ml),IL-6(11.58±2.56 pg/ml),TNF-α(26.12±6.81 pg/ml)표체현저증고(P<0.05)。결론대서자체퇴변수핵도치요교감신경간염증인자표체승고가능여반원성내장통적발생유관。
Objective The aim of the study was to observe the inflammatory response of sympathetic trunk by autol-ogous degenerative nucleus pulposus (NP)injection under fluoroscopy.Methods A total 72 rats was used,which divid-ed in 3 group:nave group,sham group and NP-treated group.24 rats in NP-treated group,autologous NP suspension was injected to right sympathetic trunk.14 days after injection,right lumbar sympathetic trunk was harvested for histo-logical assessment and ELISA quantitative determination of IL-1β,IL-6,and TNF-αprotein.Results In the NP treated group,endoneural hyperemia and intensive infiltration of inflammatory cells can be seen in section of sympathetic trunk by Hematoxylin and Eosin (HE)stained,Meantime,elevated concentration of IL-1β(20.1±4.40 pg/ml),IL-6 (26.5 ±8.9 pg/ml)and TNF-α(60.77 ± 6.37 pg/ml)by ELISA of sympathetic trunk in NP-treated group compared to naveIL-1β(8.02±1.31 pg/ml),IL-6 (10.30±1.43 pg/ml),TNF-α(24.38±4.81 pg/ml)and controlIL-1β(7.52± 2.11 pg/ml),IL-6 (11.58±2.56 pg/ml),TNF-α(26.12±6.81 pg/ml)group(P<0.05).Conclusion Elevated ex-pression of inflammatory cytokine,which caused by degenerative autologous nucleus pulposus inj ected to lumbar sympa-thetic trunk,may be related to discogenic visceral pain.
