目的：通过对4例肾淀粉样变性患者的临床及病理分析，提高对该病认识。方法回顾性分析4例经肾活检确诊为淀粉样变性病患者的临床资料及病理特点。结果4名患者年龄≥58岁，均出现肾病综合征，2例伴多系统受累；刚果红染色阳性，4例均为 AL 型，其中3例免疫荧光λ链阳性。结论伴多系统损害或激素治疗欠佳的中老年肾病综合征患者，要考虑到肾淀粉样变性病的可能；肾活检应常规行刚果红染色及电镜检查。
목적：통과대4례신정분양변성환자적림상급병리분석，제고대해병인식。방법회고성분석4례경신활검학진위정분양변성병환자적림상자료급병리특점。결과4명환자년령≥58세，균출현신병종합정，2례반다계통수루；강과홍염색양성，4례균위 AL 형，기중3례면역형광λ련양성。결론반다계통손해혹격소치료흠가적중노년신병종합정환자，요고필도신정분양변성병적가능；신활검응상규행강과홍염색급전경검사。
Objective:To describe the clinical and pathological characteristics of renal amyloidosis,to improve the diag-nosis level. Methods:The clinical and pathological data of 4 patients with renal amyloidosis were retrospect - tively ana-lyzed,who were diagnosed by renal biopsy. Results:4 patients were over 58 years old,and all presented nephrotic syndrome with severe hypoalbuminemia,2 case had more than two systems involvement. Congo red staining were positive,all patients were AL type,light chain λ were detected in 3 patients. Conclusion:Amyloidosis should be considered in elderly patients with nephrotic syndrome,who had multisystem damage or steroid resistance. Congo red stain and transmission electron mi-croscope should be included in renal biopsy.
