目的：探讨64排螺旋 CT 血管成像技术在腹主动脉瘤中的诊断及术前应用价值。方法将我院采用64排螺旋 CT 血管成像技术检查并疑似为腹主动脉瘤的53例患者进行高浓度碘对比剂血管造影检查,观察64排螺旋 CT 血管成像技术诊断的准确性。观察患者的病变累及部位、类型、大小及范围,并根据腔内隔绝术的要求对腹主动脉瘤患者进行测量和分型。结果全部患者中近肾型、肾下型、肾上型分别占11.3%(6/53)、83.0%(44/53)、5.7%(3/53)；64排螺旋 CT 检查结果准确率达96.2%,与“金标准”相比,具有相关性(r =0.9983,P <0.05)；依据三型五分法,Ⅰ型4例(7.6%,4/53)、ⅡA 型7例(13.2%,7/53)、ⅡB 型5例(9.4%,5/53)、ⅡC 型26例(49.1%,26/53)、Ⅲ型9例(17.0%,9/53)。结论64排螺旋 CT 血管成像技术具有快速、简便、精确性高的特点,是 AAA 诊断及术前评估的首选方法。

목적：탐토64배라선 CT 혈관성상기술재복주동맥류중적진단급술전응용개치。방법장아원채용64배라선 CT 혈관성상기술검사병의사위복주동맥류적53례환자진행고농도전대비제혈관조영검사,관찰64배라선 CT 혈관성상기술진단적준학성。관찰환자적병변루급부위、류형、대소급범위,병근거강내격절술적요구대복주동맥류환자진행측량화분형。결과전부환자중근신형、신하형、신상형분별점11.3%(6/53)、83.0%(44/53)、5.7%(3/53)；64배라선 CT 검사결과준학솔체96.2%,여“금표준”상비,구유상관성(r =0.9983,P <0.05)；의거삼형오분법,Ⅰ형4례(7.6%,4/53)、ⅡA 형7례(13.2%,7/53)、ⅡB 형5례(9.4%,5/53)、ⅡC 형26례(49.1%,26/53)、Ⅲ형9례(17.0%,9/53)。결론64배라선 CT 혈관성상기술구유쾌속、간편、정학성고적특점,시 AAA 진단급술전평고적수선방법。
Objective To investigate the diagnosis and preoperative application of 64-slice CT angiography in abdominal aortic aneurysm.Methods Fifty-three patients in the hospital after using 64-slice spiral CT angiography examination and suspected abdominal aortic aneurysms got high concentrations of iodine contrast agent angiography to evaluate the the diagnosis accuracy.The patients were detected for the lesion location,type,size and scope,and in accordance with the requirements of endovascular abdominal aortic aneurysm,the patients were measured and typed. Results All patients with near kidney type,under kidney type,above kidneytype accounted for 1 1.3%(6/53),83.0%(44/53),5.7%(3/53 );64-slice spiral CT proved accuracy rate 96.2%,compared with the gold standard,statistic correlation existed,the correlation coefficient (r =0.9983,P <0.05);based on three types and five points method,Ⅰ type 4 cases (7.6%,4/53 ),Ⅱ A in 7 cases (13.2%,7/53 ),Ⅱ B in 5 cases (9.4%,5/53 ),Ⅱ C type 26 cases (49.0%,26/53 ),Ⅲ 9 cases (1 7.0%,9/53 ).Conclusion 64-slice CT angiography has fast,simple,high precision characteristics,and the method is the preferred method in diagnosis and preoperative assessment of abdominal aortic aneurysm.
