目的：观察中西医结合治疗早期糖尿病肾病（ DN）的临床疗效。方法将100例DN患者随机分为2组。治疗组50例予中药灌肠联合银杏达莫注射液治疗，对照组50例予西医常规治疗。2组均治疗1个月后统计疗效、中医证候评分和实验室指标变化情况。结果治疗组总有效率高于对照组（P＜0．05），治疗组疗效优于对照组。治疗组治疗后主要中医证候评分均较本组治疗前下降（P＜0．05），而对照组治疗后只有口渴喜饮、气短懒言2个中医证候评分较治疗前下降（P＜0．05）；治疗组治疗后主要中医证候评分均低于对照组治疗后（P＜0．01）。2组治疗后空腹血糖（FPG）、尿微量白蛋白（mAlb）、血及尿β2微球蛋白（β2-MG）均较本组治疗前均降低（P＜0．05）；治疗组治疗后FPG、mAlb、血β2-MG及尿β2-MG均较对照组治疗后降低（P＜0．05）。2组治疗后血清总胆固醇（TC）、甘油三酯（TG）均较本组治疗前降低（P＜0．01）；治疗组治疗后TC、TG均较对照组治疗后降低（P＜0．01）。结论中西医结合治疗早期DN能明显提高疗效，改善患者中医证候评分，减轻或消除患者的临床症状，改善肾功能。
목적：관찰중서의결합치료조기당뇨병신병（ DN）적림상료효。방법장100례DN환자수궤분위2조。치료조50례여중약관장연합은행체막주사액치료，대조조50례여서의상규치료。2조균치료1개월후통계료효、중의증후평분화실험실지표변화정황。결과치료조총유효솔고우대조조（P＜0．05），치료조료효우우대조조。치료조치료후주요중의증후평분균교본조치료전하강（P＜0．05），이대조조치료후지유구갈희음、기단라언2개중의증후평분교치료전하강（P＜0．05）；치료조치료후주요중의증후평분균저우대조조치료후（P＜0．01）。2조치료후공복혈당（FPG）、뇨미량백단백（mAlb）、혈급뇨β2미구단백（β2-MG）균교본조치료전균강저（P＜0．05）；치료조치료후FPG、mAlb、혈β2-MG급뇨β2-MG균교대조조치료후강저（P＜0．05）。2조치료후혈청총담고순（TC）、감유삼지（TG）균교본조치료전강저（P＜0．01）；치료조치료후TC、TG균교대조조치료후강저（P＜0．01）。결론중서의결합치료조기DN능명현제고료효，개선환자중의증후평분，감경혹소제환자적림상증상，개선신공능。
Objective To observe the effect of patients with early diabetic nephropathy treated by combined therapy of Chinese and Western medicine.Methods 100 cases of DN were randomly divided into two groups.50 ca-ses in treatment group were treated by Chinese medicine enema combined with Yinxingdamo injection.50 cases in control group were given western medicine conventional treatment.The efficacy and TCM syndrome score and lab in-dex changes were observed after a month of treatment.Results The total effective rate in treatment group was higher than that in control group (P<0.05).The main TCM syndrome score after treatment was decreased as compared with that before treatment in treatment group (P<0.05).Two TCM syndrome score of thirsty like drinking, shortness of breath lazy after treatment was decreased as compared with that before treatment in control group (P<0.05).The main TCM syndrome score in treatment group was lower than that in control group after treatment (P<0.01).FPG, mALBTC andβ2 -MG in serum and urine after treatment were decreased as compared with those before treatment in two groups (P<0.05).FPG, mALBTC andβ2 -MG in serum and urine in treatment group were decreased as com-pared with those in control group after treatment (P<0.05).TC and TG after treatment were decreased as compared with those before treatment in two groups (P<0.05).TC and TG in treatment group were lower than those in control group after treatment (P<0.05).Conclusion The combined therapy of Chinese and Western medicine on patients with early diabetic nephropathy can significantly improve the efficacy, improve patient TCM syndrome score, reduce or eliminate the clinical symptoms and improve kidney function.
