目的：观察针刀结合局部阻滞治疗膝关节骨性关节炎（ KOA）的临床疗效。方法将72例KOA患者随机分为2组，各36例。针刀组采用局部阻滞后针刀切割治疗，电针组采用常规针刺加电针治疗。采用Lysholm膝关节功能评分法评价疗效，比较2组治疗前后关节晨僵、疼痛、肿胀、步行能力评分。结果2组患者治疗后关节晨僵、疼痛、步行能力、关节肿胀评分均较治疗前明显改善（均P＜0．05），针刀组较电针组改善更明显（ P＜0．01）。针刀组有效率为91．7％，电针组有效率为69．4％，针刀组有效率明显优于电针组（P＜0．01）。结论针刀结合局部阻滞治疗KOA较电针疗法能更好的改善患者的症状和体征。
목적：관찰침도결합국부조체치료슬관절골성관절염（ KOA）적림상료효。방법장72례KOA환자수궤분위2조，각36례。침도조채용국부조체후침도절할치료，전침조채용상규침자가전침치료。채용Lysholm슬관절공능평분법평개료효，비교2조치료전후관절신강、동통、종창、보행능력평분。결과2조환자치료후관절신강、동통、보행능력、관절종창평분균교치료전명현개선（균P＜0．05），침도조교전침조개선경명현（ P＜0．01）。침도조유효솔위91．7％，전침조유효솔위69．4％，침도조유효솔명현우우전침조（P＜0．01）。결론침도결합국부조체치료KOA교전침요법능경호적개선환자적증상화체정。
Objective To observe the clinical effect of needle knife combined with local anesthesia on the treatment of knee osteoarthritis ( KOA) .Methods 72 KOA patients were randomly divided into two groups,36 cases in each group.Electroacupuncture group received conventional acupuncture plus electroacupuncture.Needle-knife group received needle knife cut treatment after local block.The effect was evaluated by Lysholm knee function score method.Morning stiffness,joint pain,swelling,walking ability score before and after treatment was compared in two groups.Results Morning stiffness,joint pain,swelling,walking ability score after treatment were significantly im-proved as compared with those before treatment in two groups (P<0.05).The improvement in needle-knife group was more obvious than that in the effective rate in needle-knife group (91.7%) was higher than that in electroacu-puncture group (69.4%P<0.01).Conclusion Needle knife combined with local anesthesia can better improve the signs and symptoms of KOA patients on the treatment of knee osteoarthritis.
