目的：探索减少调配差错的措施，提高药品调配质量。方法对医院2012年至2013年住院药房《药品调配差错登记本》登记的差错进行回顾性分析。结果2年内共发生调配差错172例，其中药物品种差错53例，药物数量差错13例，剂量剂型差错25例，用法用量差错16例，包装相似差错17例，生产厂家差错12例，医师医嘱差错18例，退药差错11例，发错科室7例；2年内调配差错出现的频次呈逐渐下降趋势。结论通过住院药房调配标准化体系和调配工作各个环节标准操作规程的建立，辅以药师定期的业务培训、交流学习等机制，可大大减少调配差错的发生。
목적：탐색감소조배차착적조시，제고약품조배질량。방법대의원2012년지2013년주원약방《약품조배차착등기본》등기적차착진행회고성분석。결과2년내공발생조배차착172례，기중약물품충차착53례，약물수량차착13례，제량제형차착25례，용법용량차착16례，포장상사차착17례，생산엄가차착12례，의사의촉차착18례，퇴약차착11례，발착과실7례；2년내조배차착출현적빈차정축점하강추세。결론통과주원약방조배표준화체계화조배공작각개배절표준조작규정적건립，보이약사정기적업무배훈、교류학습등궤제，가대대감소조배차착적발생。
Objective To explore the measures for reducing the drug dispensing errors in order to improve the drug dispensing quality. Methods The drug dispensing errors registered in the Drug Dispensing Errors Registration of the inpatient pharmacy from 2012 to 2013 were performed the retrospective analysis. Results 172 cases of drug dispensing error occurred during the two years,in which 53 cases were the errors of drug species,13 cases were the errors of drug number,25 cases were the errors of dosage and dosage form,16 cases were the errors of usage and use amount,17 cases were the errors of packaging similarity,12 cases were the manufacturer errors,18 cases were the medical order errors,11 cases were the drug repercussion errors and 7 cases were the errors for dispensing medicine to wrong department;the dispensing errors frequency showed the gradually decreasing trend within the two years. Conclusion The occurrence of drug dispensing errors is greatly reduced by establishing the inpatient pharmacy dispensing standardized system and the standard operating procedures of the various links in the drug dispensing work and supplementing by the mechanisms of the pharmacist regular professional training,exchange and learning.
