目的:探讨实施介入静脉滤器术及机械性溶栓术后,低分子肝素治疗和预防骨科术后下肢深静脉血栓的疗效。方法选取2010年1月至2013年1月收治的60例骨科手术后下肢深静脉血栓患者,均实施静脉滤器植入术和超声引导下的机械性溶栓术,随机分为两组,各30例。术后对照组采用常规治疗,观察组采用低分子肝素治疗。并观察比较两组患者血小板数量、抗X因子活性峰值、一氧化氮及内皮素-1水平,以及缓解率、复发率及生存率。结果观察组血小板数量及内皮素-1水平明显低于对照组( P<0.05),一氧化氮水平及抗 X因子活性峰值显著高于对照组( P<0.05)。观察组复发率明显低于对照组( P<0.05),缓解率和生存率显著高于对照组( P<0.05)。结论介入联合低分子肝素可有效治疗和预防下肢深静脉血栓,可能通过升高抗X因子、一氧化氮水平及降低内皮素-1水平而发挥抗凝作用,可提高生存率、降低复发率,值得临床推广。

목적:탐토실시개입정맥려기술급궤계성용전술후,저분자간소치료화예방골과술후하지심정맥혈전적료효。방법선취2010년1월지2013년1월수치적60례골과수술후하지심정맥혈전환자,균실시정맥려기식입술화초성인도하적궤계성용전술,수궤분위량조,각30례。술후대조조채용상규치료,관찰조채용저분자간소치료。병관찰비교량조환자혈소판수량、항X인자활성봉치、일양화담급내피소-1수평,이급완해솔、복발솔급생존솔。결과관찰조혈소판수량급내피소-1수평명현저우대조조( P<0.05),일양화담수평급항 X인자활성봉치현저고우대조조( P<0.05)。관찰조복발솔명현저우대조조( P<0.05),완해솔화생존솔현저고우대조조( P<0.05)。결론개입연합저분자간소가유효치료화예방하지심정맥혈전,가능통과승고항X인자、일양화담수평급강저내피소-1수평이발휘항응작용,가제고생존솔、강저복발솔,치득림상추엄。
Objective To investigate the effect of low molecular heparin in treating and preventing lower extremity deep venous thrombosis after implementation of interventional vena cava filter operation and mechanical thrombolysis operation. Methods 60 patients with total hip arthroplasty(THA)admitted to our hospital from January 2010 to January 2013 were implemented the interventional vena cava filter implantation and ultrasound-guided mechanical thrombolysis operation,and randomly divided into 2 groups,30 Cases in each group. The control group was treated with postoperative conventional therapy,while the observation group was treated with low molecular heparin. The platelet count,activity peak value of anti-X factor,nitric oxide and endothelin-1 level,remission rate,recurrence rate and survival rate were observed and compared between the two groups. Results The platelet count and endothelin-1 level in the observa-tion group were significantly lower than those in the control group( P < 0. 05),while the nitric oxide level and the activity peak value of anti-X factor were significantly higher than those in the control group( P < 0. 05). The recurrence rate of the observation group was significantly lower than that of the control group( P < 0. 05),while the remission rate and the survival rate were significantly high-er than those in the control group( P < 0. 05). Conclusion Intervention combined with postoperative low molecular heparin can effec-tively treat and prevent the postoperative deep vein thrombosis and may play the anticoagulant role by elevating the anti-X factor and nitric oxide levels and decreasing the endothelin-1 level,which can improve the survival rate,reduce the recurrence rate and be worthy of clinical promotion.
