目的:探讨肿瘤标志物神经特异性烯醇化酶(NSE)、细胞角蛋白(CYFRA21-1)、癌胚抗原(CEA)、糖类抗原125(CA125)、糖类抗原153(CA153)和铁蛋白(Ferritin)对肺癌的诊断价值。方法采用电化学发光法分别检测151例肺癌(肺癌组)、100例肺良性疾病患者(肺良性疾病组)及50例健康对照组的血清 NSE、CYFRA21-1、CEA、CA125、CA153、Ferritin 水平并进行分析。结果肺癌组患者6种肿瘤标志物水平明显高于肺良性疾病组和健康对照组,差异有统计学意义(P <0.05)。小细胞肺癌的 NSE 水平高于鳞癌和腺癌,差异有统计学意义(P <0.05),而鳞癌和腺癌 CYFRA21-1水平高于小细胞肺癌,差异有统计学意义(P <0.05)。6种肿瘤标志物中 NSE 的灵敏度最高,为82.8%。6项肿瘤标志物中3项阳性时,肺癌组阳性率较高,假阳性显著降低。结论血清肿瘤标志物的水平和阳性率与肺癌的病理类型有关。联合检测血清肿瘤标志物可提高早期诊断肺癌的灵敏度和特异度,首选 CEA、NSE 和 CYFRA21-13种肿瘤标志物联合检测。

목적:탐토종류표지물신경특이성희순화매(NSE)、세포각단백(CYFRA21-1)、암배항원(CEA)、당류항원125(CA125)、당류항원153(CA153)화철단백(Ferritin)대폐암적진단개치。방법채용전화학발광법분별검측151례폐암(폐암조)、100례폐량성질병환자(폐량성질병조)급50례건강대조조적혈청 NSE、CYFRA21-1、CEA、CA125、CA153、Ferritin 수평병진행분석。결과폐암조환자6충종류표지물수평명현고우폐량성질병조화건강대조조,차이유통계학의의(P <0.05)。소세포폐암적 NSE 수평고우린암화선암,차이유통계학의의(P <0.05),이린암화선암 CYFRA21-1수평고우소세포폐암,차이유통계학의의(P <0.05)。6충종류표지물중 NSE 적령민도최고,위82.8%。6항종류표지물중3항양성시,폐암조양성솔교고,가양성현저강저。결론혈청종류표지물적수평화양성솔여폐암적병리류형유관。연합검측혈청종류표지물가제고조기진단폐암적령민도화특이도,수선 CEA、NSE 화 CYFRA21-13충종류표지물연합검측。
Objective To investigate the value of serum tumor markers neuron specific enolase (NSE),cytokeratin 19 fragment (CYFRA21-1),carcinoembryonic antigen (CEA),carbohydrate antigen 125 (CA125 ),carbohydrate antigen 153 (CA153)and fer-ritin in diagnosing lung cancer.Methods The serum levels of NSE,CYFRA21-1,CEA,CA125,CA153 and ferritin were detected by the electrochemiluminescence method in 151 cases of lung cancer(lung cancer group),100 cases of benign lung disease (benign dis-ease group)and 50 cases of healthy control group.The detection results were analyzed.Results The serum levels of NSE,CY-FRA21-1,CEA,CA125,CA153 and ferritin in the lung cancer group were significantly higher than those in the benign lung disease group and the healthy control group with statistical differences (P <0.05);the NSE level in small cell lung cancer was higher than that in squamous carcinoma and adenocarcinoma with statistical difference(P <0.05).The CYFRA21-1level in squamous carcinoma and adencarcinoma was higher than that in small cell lung cancer with statistical difference (P <0.05).The sensitivity of NSE was highest in six tumor markers,which was 82.8%.In 6 tumor markers,3 markers were positive,the positive rate of lung cancer was higher,while the false positive rate was significantly reduced.Conclusion The levels and positive rates of serum tumor markers are associated with the pathological type of lung cancer.The combined detection of serum tumor markers can increase the sensitivity and accuracy in early diagnosis of lung cancer.The combination measurement of CEA,NSE and CYFRA21-1 is the first choice.
