目的:探讨逆行面动脉供血下颌下腺转移治疗重症干眼症的解剖学证据。方法6例正常成人头颈标本,测量面动脉出下颌下腺沟点至上、下唇动脉分叉处长度、下颌下腺导管长度和同侧口角至外眦角距离,测量下颌下腺长度、宽度和厚度。以面动脉发出下唇动脉点为圆心,向同侧眶周转移下颌下腺。结果面动脉出下颌下腺沟点至上、下唇动脉分叉处长度(6.8±1.2)c m、下颌下腺导管长度(6.4±0.7)c m,同侧口角至外眦角距离(8.6±0.8)c m,下颌下腺长(3.8±0.6)c m,宽(2.7±0.3)c m,厚(1.6±0.4)c m。以面动脉为蒂,下颌下腺可无张力地转移到眶外下区。结论以面动脉为蒂下颌下腺转移治疗重症干眼症手术可行。

목적:탐토역행면동맥공혈하합하선전이치료중증간안증적해부학증거。방법6례정상성인두경표본,측량면동맥출하합하선구점지상、하진동맥분차처장도、하합하선도관장도화동측구각지외자각거리,측량하합하선장도、관도화후도。이면동맥발출하진동맥점위원심,향동측광주전이하합하선。결과면동맥출하합하선구점지상、하진동맥분차처장도(6.8±1.2)c m、하합하선도관장도(6.4±0.7)c m,동측구각지외자각거리(8.6±0.8)c m,하합하선장(3.8±0.6)c m,관(2.7±0.3)c m,후(1.6±0.4)c m。이면동맥위체,하합하선가무장력지전이도광외하구。결론이면동맥위체하합하선전이치료중증간안증수술가행。
Objective To explore the anatomical evidence of submandibular gland transfer with reverse blood supply for severe xerophthalmia patients. Method 6 specimens of adult head and neck were dissected. The length of the facial artery, the Wharton duct,the submandibular gland and the distance from the corner of mouth to the ipsileteral outer canthus were measured. The freed submandibular gland was tried to transfer to the infra-external orbital region. Results The length of the facial artery, the Wharton duct,and the distance from the corner of mouth to the ipsileteral outer canthus was (6.8±1.2)cm, (6.4±0.7)cm, (8.6±0.8) cm respectively. The submandibular gland was (3.8±0.6)cm in length, (2.7±0.3)cm in width, and (1.6±0.4)cm in thickness. The submandibular gland with pedicle of facial artery can be transferred to the infra-external orbital region. Conclusion Transfer of the submandibular gland with pedicle of reverse facial artery was available for severe xerophthalmia.
