目的：探讨后腹腔镜在肾癌根治术中的应用,评价其安全性及临床疗效。方法2011年11月~2013年11月我院行后腹腔镜肾癌根治术32例。结果32例手术均获得成功，手术时间90~150min，平均115min，术中平均出血量约75ml，术中无大出血，术中术后无输血，无脏器损伤、气体栓塞、皮下气肿等并发症，无二次手术病例，术后住院时间5~8d，肠道功能恢复1~2d，术后随访3~24个月，平均随访8个月，无肿瘤复发及转移。结论后腹腔镜肾癌根治术具有切口小、出血少、恢复快、并发症少及安全有效的优点，具有良好的应用前景。
목적：탐토후복강경재신암근치술중적응용,평개기안전성급림상료효。방법2011년11월~2013년11월아원행후복강경신암근치술32례。결과32례수술균획득성공，수술시간90~150min，평균115min，술중평균출혈량약75ml，술중무대출혈，술중술후무수혈，무장기손상、기체전새、피하기종등병발증，무이차수술병례，술후주원시간5~8d，장도공능회복1~2d，술후수방3~24개월，평균수방8개월，무종류복발급전이。결론후복강경신암근치술구유절구소、출혈소、회복쾌、병발증소급안전유효적우점，구유량호적응용전경。
Objective To investigatethe use of retroperitoneal aparoscopicfor radical nephrectomy, and evaluateits securityand ef icacy. Methods 32 patientsreceivedthetreatmentof retroperitoneal aparoscopicradical nephrectomyfrom November2011 to November2013 inour hospital.Results Al the 32 operations weresuccessful.The operation timewas 90~150 minutes, and the average timewas 115 minutes. The average intraoperative blood loss was about 75 ml, and no massive haemor hageoccur ed. None of the 32 cases receivedblood transfusionduringor after the operation. No viscera damage, gas embolism, subcutaneous emphysema and other complicationsoccur ed. None of the 32 cases received secondary surgery.The postoperative hospital staywas 5-8 days. The recovery time of intestinal functionwas 1~2 days. The postoperative fol ow-up timewas 3~24 months, and the average time was 8 months. No tumorrecurrence and metastasisoccurred.Conclusion Retroperitoneal aparoscopicradical nephrectomyhad the advantages ofsmal erincision, less bleeding, faster recovery, fewer complications, saferand more ef ective. It had a good application prospect.
