目的:探讨同型半胱氨酸(Hcy)、血清胱抑素C(CysC)、糖化血红蛋白(HbA1c)和尿微量白蛋白(U-mAlb)联合检测对糖尿病早期肾脏损伤的诊断价值。方法分别检测122例2型糖尿病(T2DM)患者[尿蛋白阴性(A组)62例,尿蛋白阳性(B组)60例]和健康体检者(NC组)65例的血清Hcy、CysC、HbA1c和U-mAlb的含量。结果 T2DM患者Hcy、CysC、HbA1c、U-mAlb的水平明显高于NC组(P<0.01),且B组较A组升高明显(P<0.01)。结论血清Hcy、CysC、HbA1c与U-mAlb的水平与T2DM患者的早期肾损伤程度密切相关,联合检测对于糖尿病肾病的早期诊断、早期治疗及监测有重要意义。

목적:탐토동형반광안산(Hcy)、혈청광억소C(CysC)、당화혈홍단백(HbA1c)화뇨미량백단백(U-mAlb)연합검측대당뇨병조기신장손상적진단개치。방법분별검측122례2형당뇨병(T2DM)환자[뇨단백음성(A조)62례,뇨단백양성(B조)60례]화건강체검자(NC조)65례적혈청Hcy、CysC、HbA1c화U-mAlb적함량。결과 T2DM환자Hcy、CysC、HbA1c、U-mAlb적수평명현고우NC조(P<0.01),차B조교A조승고명현(P<0.01)。결론혈청Hcy、CysC、HbA1c여U-mAlb적수평여T2DM환자적조기신손상정도밀절상관,연합검측대우당뇨병신병적조기진단、조기치료급감측유중요의의。
Objective To explore the diagnostic value of joint detection of homocystein (Hcy), serum cystatin C (CysC), glycosylated hemoglobin (HbA1c) and urine microalbumin (U-mAlb) in diagnosis of early diabetic renal impairment. Methods The levels of serum Hcy, CysC, HbA1c and U-mAlb were detected in 65 health people (NC group) and 122 patients with type 2 diabets mellitus (T2DM) [62 patients with negative urinary protein (group A), 60 patients with positive urinary protein (group B)]. Results The levels of Hcy, CysC,HbA1c and U-mAlb in T2DM patients were obviously higher than those of NC group (P<0.01), and those levels increased more significantly in group B than in group A (P<0.01). Conclusion The levels of serum Hcy, CysC, HbA1c and U-mAlb are closely related to the early renal impairment in T2DM. The joint detection is important for the early diagnosis, control and monitoring of diabetic nephropathy.
