目的:探讨食管鳞状细胞癌(ESCC)患者血浆 miR -200c 和 miR -155的表达水平及二者的临床意义。方法选取2012年9月-2013年5月在郑州大学第一附属医院确诊为 ESCC 的患者33例(ESCC 组)和同期在本院行体检健康者30例(对照组)。采用实时定量反转录聚合酶链反应(qRT - PCR)测定两组受检者血浆 miR -200c、miR -155水平,分析二者诊断 ESCC 的价值,并探讨二者与 ESCC 患者各临床病理指标间的关系。结果(1)ESCC组患者血浆 miR -200c、miR -155水平均高于对照组〔(13.87±2.27)与(2.43±3.04),(8.09±1.97)与(1.77±2.74);P <0.05〕。(2)受试者工作特征(ROC)曲线分析显示,血浆 miR -200c 和 miR -155诊断 ESCC 的 ROC曲线下面积( AUC)分别为0.853〔95% CI(0.774,0.931),灵敏度为79%,特异度为88%〕和0.771〔95% CI (0.674,0.867),灵敏度为69%,特异度为81%〕。(3)ESCC 患者血浆 miR -200c 水平与病理分期及是否转移有关(P <0.05);而与性别、年龄、分化程度、是否手术无关(P >0.05)。ESCC 患者血浆 miR -155水平与各临床病理指标均无关(P >0.05)。结论 ESCC 患者血浆 miR -200c、miR -155较健康者表达水平升高,可作为诊断 ESCC 潜在的生物标志物;血浆 miR -200c 水平与 ESCC 病理进程相关。

목적:탐토식관린상세포암(ESCC)환자혈장 miR -200c 화 miR -155적표체수평급이자적림상의의。방법선취2012년9월-2013년5월재정주대학제일부속의원학진위 ESCC 적환자33례(ESCC 조)화동기재본원행체검건강자30례(대조조)。채용실시정량반전록취합매련반응(qRT - PCR)측정량조수검자혈장 miR -200c、miR -155수평,분석이자진단 ESCC 적개치,병탐토이자여 ESCC 환자각림상병리지표간적관계。결과(1)ESCC조환자혈장 miR -200c、miR -155수평균고우대조조〔(13.87±2.27)여(2.43±3.04),(8.09±1.97)여(1.77±2.74);P <0.05〕。(2)수시자공작특정(ROC)곡선분석현시,혈장 miR -200c 화 miR -155진단 ESCC 적 ROC곡선하면적( AUC)분별위0.853〔95% CI(0.774,0.931),령민도위79%,특이도위88%〕화0.771〔95% CI (0.674,0.867),령민도위69%,특이도위81%〕。(3)ESCC 환자혈장 miR -200c 수평여병리분기급시부전이유관(P <0.05);이여성별、년령、분화정도、시부수술무관(P >0.05)。ESCC 환자혈장 miR -155수평여각림상병리지표균무관(P >0.05)。결론 ESCC 환자혈장 miR -200c、miR -155교건강자표체수평승고,가작위진단 ESCC 잠재적생물표지물;혈장 miR -200c 수평여 ESCC 병리진정상관。
Objective To investigate the expression level and clinical significance of plasma miR - 200c and miR - 155 in patients with esophageal squamous cell carcinoma(ESCC). Methods 33 ESCC patients confirmed in the First Affitiated Hospital of Zhengzhou University( ESCC group)and 30 healthy people underwent physical examination( control group)from September 2012 to May 2013 were selected. Quantitative real - time polymerase chain reaction(qRT - PCR)was used to detect the plasma miR - 200c and miR - 155 levels,and the value of the two in diagnosing ESCC was analyzed,as well as the relationship between the two indicators and clinical indexes of ESCC patients. Results The expression levels of plasma miR- 200c and miR - 155 in ESCC group were significantly higher than that in control group〔(13. 87 ± 2. 27)vs. (2. 43 ± 3. 04);(8. 09 ± 1. 97)vs. (1. 77 ± 2. 74);P < 0. 05〕. The ability of plasma miR - 200c and miR - 155 to discriminate the ESCC from those controls were evaluated by the ROC curve with an AUC of 0. 853〔95% CI(0. 774,0. 931),sensitivity 79% ,spe-cificity 88% 〕and 0. 771〔95% CI(0. 674,0. 867),sensitivity 69% ,specificity 81% 〕respectively. Plasma miR - 200c lev-els were not associated with gender,age,differentiation degree and operation( P > 0. 05),but were related to pathological stage and metastasis(P < 0. 05);plasma miR - 155 levels were not associated with any of the clinical pathological indicators collected(P > 0. 05). Conclusion The expression levels of plasma miR - 200c and miR - 155 are significantly increased in patients with ESCC,and they could serve as potential diagnostic biomarkers for ESCC. The plasma miR - 200c level is associated with the pathological process of ESCC.
