目的观察创伤性脑损伤患者亚急性期无创监测脑氧饱和度变化及影响脑氧饱和度的因素。方法选择2007-11~2008-07苏州大学附属第二医院高新区医院脑外科的患者40例,正常对照组选取健康成年人30例。患者的一般资料包括性别、年龄、血压、GCS评分、住院天数等。采用红外光谱技术( near-infrared spectroscopy, NIR),颅内氧饱和度监测仪测定脑组织氧饱和度( regional oxygen saturation, rSO2)的变化,在脑氧饱和度测定同时抽取桡动脉血,测定血气分析。同时记录心率、无创血压、经皮氧饱和度和呼吸频率等。结果亚急性创伤性脑损伤患者的rSO2较健康对照组显著降低(P<0.01)。两组左右间rSO2比较差异无统计学意义(P>0.01)。rSO2和血红蛋白呈正相关(r=0.899,P<0.01)。 rSO2和年龄呈负相关(r=0.845,P<0.01)。结论创伤性脑损伤亚急性期患者存在脑氧代谢紊乱,rSO2受年龄和血红蛋白的影响。

목적관찰창상성뇌손상환자아급성기무창감측뇌양포화도변화급영향뇌양포화도적인소。방법선택2007-11~2008-07소주대학부속제이의원고신구의원뇌외과적환자40례,정상대조조선취건강성년인30례。환자적일반자료포괄성별、년령、혈압、GCS평분、주원천수등。채용홍외광보기술( near-infrared spectroscopy, NIR),로내양포화도감측의측정뇌조직양포화도( regional oxygen saturation, rSO2)적변화,재뇌양포화도측정동시추취뇨동맥혈,측정혈기분석。동시기록심솔、무창혈압、경피양포화도화호흡빈솔등。결과아급성창상성뇌손상환자적rSO2교건강대조조현저강저(P<0.01)。량조좌우간rSO2비교차이무통계학의의(P>0.01)。rSO2화혈홍단백정정상관(r=0.899,P<0.01)。 rSO2화년령정부상관(r=0.845,P<0.01)。결론창상성뇌손상아급성기환자존재뇌양대사문란,rSO2수년령화혈홍단백적영향。
Objective The aim of this study is to investigate the regional transcranial oxygen saturation ( rSO2 ) in patients with sub-acute traumatic brain injury ( TBI) , and the factors affecting a variability of rSO2 values.Methods This observational study was carried in a teaching hospital.There were two groups in this study.The healthy group involved 30 healthy people, and the TBI group included 40 TBI patients.The rSO2 was measured by Somanetics INVOS 5100 monitor and the arterial blood was sampled to measure blood gas analysis.The healthy people were measured by INVOS 5100 monitor for the rSO2 only.Results The parameter of rSO2 in patients with sub-acute traumatic brain injury was significantly decreased compared with healthy people ( P <0.01).A significant negative correlation between the rSO2 values and age and a positive correlation between the rSO2 values and hemoglobin concentration were observed (P<0.01).Conclusion ①There is a disorder of oxygen metabolism in patients with sub-acute TBI;②These data indicate that patient age and hemoglobin concentration can affect rSO2 values.
