目的:分析青年脑梗死患者的血管病变表现及分布情况及其与脑梗死类型之间的关系。方法收集本院2006年10月~2013年5月住院的经头颅影像学明确诊断的青年脑梗死患者,并依据影像学表现进行脑梗死分型,全脑血管造影评估血管病变状况。结果共收集符合入选标准的患者48例,其中男39例,女9例,青年脑梗死的影像学主要表现为部分性前循环梗死和腔隙性脑梗死2种(34/48),病变血管主要分布于前循环,主要累及颈内动脉颅内外段以及大脑中动脉。血管病变主要表现为狭窄和闭塞,同时多血管病变、血管串联病变情况表现突出。结论青年脑梗死患者颈内动脉系统累及比例高,多血管病变以及单根血管多处病变突出,血管病变主要表现为狭窄和闭塞、新生血管明显。

목적:분석청년뇌경사환자적혈관병변표현급분포정황급기여뇌경사류형지간적관계。방법수집본원2006년10월~2013년5월주원적경두로영상학명학진단적청년뇌경사환자,병의거영상학표현진행뇌경사분형,전뇌혈관조영평고혈관병변상황。결과공수집부합입선표준적환자48례,기중남39례,녀9례,청년뇌경사적영상학주요표현위부분성전순배경사화강극성뇌경사2충(34/48),병변혈관주요분포우전순배,주요루급경내동맥로내외단이급대뇌중동맥。혈관병변주요표현위협착화폐새,동시다혈관병변、혈관천련병변정황표현돌출。결론청년뇌경사환자경내동맥계통루급비례고,다혈관병변이급단근혈관다처병변돌출,혈관병변주요표현위협착화폐새、신생혈관명현。
Objective To analyze the correlation between the distributions of vascular lesions caused by cerebral infarction (CI)in young adults ,different types of CI,provide the corresponding cause analysis and medical treatment.Methods Young adults with CI perform digital subtraction cerebral angiography (DSCA) evaluation from Oct.2006 to May 2013 in the Lanzhou General Hospital of the Lanzhou Military Area Com-mand.Results 48 samples (39 male cases and 9 female cases)were collected and analyzed.The classification of imaging shows that partial anterior circulation infarct (PACI)and lacunar infarction accounted for 34 instances among the total 48 cases.Most of vascular lesion are stenosis and occlusion.They mainly appeared in anterior circulation.outer intracranial segment of internal carotid artery and middle cerebral artery were involved.Mul-tivessel and tandem vascular lesions were notable.Conclusions CI in young adults and the lesions in internal carotid artery were highly correlated.Treatment in acute phase should pay attention to blood pressure fluctua-tions,esp.low blood pressure,which will exacerbate the CI due to the decompressed perfusion pressure.
