目的:探讨医疗纠纷发生的原因,提出相应的防范对策和建议,以期提高医院的管理水平。方法采用回顾性流行病学调查方法,对上海市某综合性医院2011-2013年的273例医疗纠纷进行分析。结果医疗纠纷涉及的主要人员中副高职称以上者占56.8%;外科医疗纠纷发生的比率最高,为40.3%,其次是内科,为27.8%;医源性投诉的比例最高,为72.2%,其中因服务态度导致的医疗纠纷比例为34%;医疗纠纷的处理以选择协商途径的比例最高,为89.7%。结论医源性投诉是医疗纠纷的主要原因,其中服务态度引发的医疗纠纷比率最高。为有效预防医疗纠纷的发生,应加大科室管理力度,健全和落实各项医疗规章制度,引入人民调节机制,积极借鉴国外相关医患矛盾处理制度,提高医疗技术水平和服务质量。

Objective To explore the reasons of medical disputes and put forward some countermeasures and advice in order to enhance the management level of hospital.Methods Survey method of retrospective epide-miology was used to analyze 273 medical dispute cases of one comprehensive hospital from 2011 to 2013 in Shang-hai .Results Most of the medical staff involved in the medical disputes were senior employees. The ratio of medi-cal disputes of surgery departments was 40.3%, it took the first place in all departments.This ratio of internal medical department took the second place, it was 27.8%.The ratio of medical dispute caused by hospital factors took the first place, it was 72.2%.In all medical dispute factors, the ratio caused by service attitude of medical staff was 34%.89.7%of medical dispute cases were resolved through consultation.Conclusions The main rea-son of medical dispute was hospital factors.The ratio of medical disputes caused by service attitude took the first place.In order to prevent medical dispute effectively, we should enhance hospital management, and implement medical system, and introduce the system of people mediation, and refer to the relative systems of other countries, and improve medical technology and service quality.
