目的：分析医院退换药现象产生的原因，针对性地提出应对措施，保障患者用药安全有效。方法对我院2013年退换药情况进行回顾性研究分析。结果在退换药品与医患关系方面，医生因素占47.6%，患者因素占46.0%；在退换药品与药物属性的关系方面，药品不良反应高居榜首，占57.3%。结论应强化咨询指导，提升药学服务能力；严格制度，规范医务人员诊疗行为；加强沟通，提高患者用药依从性。尽量避免或减少退换药现象的发生。

목적：분석의원퇴환약현상산생적원인，침대성지제출응대조시，보장환자용약안전유효。방법대아원2013년퇴환약정황진행회고성연구분석。결과재퇴환약품여의환관계방면，의생인소점47.6%，환자인소점46.0%；재퇴환약품여약물속성적관계방면，약품불량반응고거방수，점57.3%。결론응강화자순지도，제승약학복무능력；엄격제도，규범의무인원진료행위；가강구통，제고환자용약의종성。진량피면혹감소퇴환약현상적발생。
Objective To analyze the reasons for the phenomenon of drugs return and exchange in hospital,and to propose the countermeasures to ensure safety and effectiveness of medication for patients.Methods Retrospective study analysis was conducted regarding the causes of drugs return and exchange in our hospital in 2 0 1 3 .Results In the aspect of doctor-patient relationship,the doctor factors accounted for 4 7.6 % ,and the patient factors accounted for 4 6.0 % . In the aspect of drug properties, adverse drug reactions ranked the first, accounting for 5 7.3 % . Conclusion We should strengthen consultation and improve pharmacy service capabilities,develop more strict systems, standardize the practices of the medical staff,and enhance communication to ensure patients'medication safety.With the above countermeasures,the occurrence of drugs return and exchange phenomenon might be avoided or reduced.
