目的:观察前臂游离皮瓣联合异种脱细胞真皮基质(H-ADM)修复口腔肿瘤术后组织缺损的临床效果。方法36例口腔肿瘤患者随机分为观察组和对照组各18例,对照组采用游离前臂皮瓣修复术后组织缺损,观察组采用游离前臂皮瓣联合H-ADM修复术后组织缺损。比较两组术后皮瓣存活率、语音功能、吞咽功能及术后供区恢复情况。结果观察组皮瓣存活率为100.00%,优于对照组的66.67%;观察组语音功能、吞咽功能以及供区恢复情况均优于对照组,所有差异均具有统计学意义(P<0.05)。结论应用游离前臂皮瓣联合H-ADM修复口腔肿瘤术后组织缺损是较为理想的方法,可恢复和改善患者的外形和生理功能,保持口腔和舌的基本功能,有效提高患者的生存质量。

목적:관찰전비유리피판연합이충탈세포진피기질(H-ADM)수복구강종류술후조직결손적림상효과。방법36례구강종류환자수궤분위관찰조화대조조각18례,대조조채용유리전비피판수복술후조직결손,관찰조채용유리전비피판연합H-ADM수복술후조직결손。비교량조술후피판존활솔、어음공능、탄인공능급술후공구회복정황。결과관찰조피판존활솔위100.00%,우우대조조적66.67%;관찰조어음공능、탄인공능이급공구회복정황균우우대조조,소유차이균구유통계학의의(P<0.05)。결론응용유리전비피판연합H-ADM수복구강종류술후조직결손시교위이상적방법,가회복화개선환자적외형화생리공능,보지구강화설적기본공능,유효제고환자적생존질량。
Objective To observe the clinical effect of free forearm flap combined with heterogeneous acellular dermal matrix (H-ADM) in reconstructing postoperative tissue defects of oral tumor. Methods 36 cases of patients with oral tumor were selected and randomly divided into observation group and control group, with 18 cases in each group. The control group was given free forearm flap, the observation group was given free forearm flap combined with H-ADM. The flap survival rate, voice function, swallowing function and postoperative recovery of supply area were compared. Results The flap survival rate of the observation group was 100.00%, better than 66.67%of the control group;and the voice function, swallowing function and postoperative recovery of supply area of the observation group were better than those of the control group, with significant difference (P <0.05). Conclusions The application of free forearm flap combined with H-ADM is an ideal method to repair postoperative tissue defects of oral tumor, which will recover and improve patients' appearance and physiological functions, maintain the basic functions of mouth and tongue and effectively improve patients' quality of life.
