下胫腓联合损伤是踝关节骨折的常见伴发伤，下胫腓联合损伤可致踝关节稳定性丧失，早期正确的诊断和治疗是预后良好的关键。处理不当易造成踝关节慢性不稳定和创伤性关节炎的发生。目前在下胫腓联合损伤的诊断和治疗上存在较多争议，主要涉及下胫腓联合损伤后稳定程度的判定及固定方式的选择。本文对下胫腓联合损伤从解剖学、生物力学、损伤机制、诊断及治疗进展方面进行综述。

The tibiofibular syndesmosis injuries occur in ankle fractures and in order to the loss of stability of ankle joint. Early and accurate diagnosis and treatment is the key to archive satisfactory outcome. Incorrect treatment may lead to chronic ankle instability and osteoarthritis. Some controversy still exists regarding the evaluation and treatment of syndesmotic injuries,particularly the determination of stability of the injured syndesmosis and timing and pattern of syndesmotic fixation. Here we review the current information about anatomy,mechanism of injury and biomechanics syndesmosis. In addition,the diagnosis and advances in therapy of syndesmotic injuries are discussed.
