我国农村居民面临着众多健康风险，承担着沉重的疾病经济负担。而目前我国农村居民的健康风险管理采用的是医疗保险模式，即依赖现有的医疗保险制度侧重于事后的医疗费用补偿，不能从根本上解决这一问题，有必要探索从医疗保险模式转变为“预防为主、防治结合”的健康管理模式。首先运用二步模型法和人力资本法，测算了我国农村居民16类主要疾病的直接和间接经济负担，并从总体和结构上对其进行了评价分析；其次，阐明了农村医疗保险制度在减轻疾病经济负担方面的不足及根源；最后，论述了从医疗保险模式转向健康管理模式的必然性，并探索了健康管理的具体步骤。

T he rural residents in our country face various health risks and bear heavy economic burden of disease .Since the current health care system cannot solve this problem fundamentally by simplex compensations for medical expenses ,it is necessary to explore a new health management mode combining disease prevention and control .This article firstly measures the direct and indirect economic burden caused by 16 main kinds of diseases ,and analyzes the results from the sum and structure .Secondly ,this article clarifies that the current rural medical insurance system is not enough to reduce the rural residents ’ economic burden at root . Finally , this article discusses the necessity of developing health management mode ,and explores the concrete steps of health management .
