目的：对甲状腺微小乳头状癌的临床病理特点以及相关的诊断要点进行分析探讨。方法对26例甲状腺微小乳头状癌患者的临床资料和组织学形态进行回顾性分析和研究。结果26例甲状腺微小乳头状癌呈结节状或纤维瘢痕状，边界不清，灰白色，质中等，癌组织有纤维血管轴心的复杂分支乳头状结构及毛玻璃核、核内胞浆假包涵体、核沟，纤维间质反应及砂粒体。结论甲状腺微小乳头状癌具有特征性的组织学特征，早期正确病理诊断对临床的治疗和预后的判断有指导意义。
목적：대갑상선미소유두상암적림상병리특점이급상관적진단요점진행분석탐토。방법대26례갑상선미소유두상암환자적림상자료화조직학형태진행회고성분석화연구。결과26례갑상선미소유두상암정결절상혹섬유반흔상，변계불청，회백색，질중등，암조직유섬유혈관축심적복잡분지유두상결구급모파리핵、핵내포장가포함체、핵구，섬유간질반응급사립체。결론갑상선미소유두상암구유특정성적조직학특정，조기정학병리진단대림상적치료화예후적판단유지도의의。
Objective To investigate the clinic pathological features of papillary thyroid microcarcinoma so as to improve its diagnostic level. Methods The clinic pathological data and the histological features in 26 cases of papillary thyroid microcarcinoma in our hospital were retrospectively analyzed. Results Gray nodules of papillary thyroid microcarcinoma were seen to the unaided eye,which much was located in thyroid essence inside with the quality of a material medium,had unclear limit around thyroid tissue without obvious envelope,which was partly radioactive scar and partly gritty texture;papillary carcinoma was found by microscopy which growed around the outside the spread of thyroid tissues,cancer cells were arranged in papillary or follicle crumb;Fiber stromal reac-tion and sand body were seen in the multistage branch true papillary structure and ground glass,intranuclear pseudoinclusion,nuclear grooves of fiber axis of blood vessels. Conclusion Papillary thyroid microcarcinoma has distinctive histologic features. The early correct pathological diagnosis of papillary thyroid micro-carcinoma has the guidance significance to the clinical treatment and the prognostic judgement.
