目的探讨微型钛板塑形内固定整复术,在三维CT重建技术的指导下,治疗眼眶骨折的临床效果。方法术前行横断面、冠状面二维CT扫描和三维CT重建,根据三维CT重建图像,采用微型钛板塑形内固定,对17例眼眶骨折患者进行骨折复位及眼眶骨缺损整复手术。结果17例术后复视及眼球运动障碍均消失,眼球内陷纠正,视力无损害,且面部外观满意,无植入体轮廓阴影或阶梯状畸形。CT扫描复查眼眶骨折及缺损处修复良好,软组织完全回纳,钛板无移位。术后随访3个月~1年,钛板无排斥反应、移位、感染及其他不良反应。结论微型钛板塑形内固定整复术,充分利用钛板具有的良好生物相容性、坚固强度大、可塑形但又不易变形、体内植入可永久留置等特点,达到眶缘结构良好的固定作用。同时在三维CT重建技术的指导下,眶骨骨折诊断更准确,手术更精确,盲目操作风险性更小,手术时间缩短,大大提高了手术安全性。
目的探討微型鈦闆塑形內固定整複術,在三維CT重建技術的指導下,治療眼眶骨摺的臨床效果。方法術前行橫斷麵、冠狀麵二維CT掃描和三維CT重建,根據三維CT重建圖像,採用微型鈦闆塑形內固定,對17例眼眶骨摺患者進行骨摺複位及眼眶骨缺損整複手術。結果17例術後複視及眼毬運動障礙均消失,眼毬內陷糾正,視力無損害,且麵部外觀滿意,無植入體輪廓陰影或階梯狀畸形。CT掃描複查眼眶骨摺及缺損處脩複良好,軟組織完全迴納,鈦闆無移位。術後隨訪3箇月~1年,鈦闆無排斥反應、移位、感染及其他不良反應。結論微型鈦闆塑形內固定整複術,充分利用鈦闆具有的良好生物相容性、堅固彊度大、可塑形但又不易變形、體內植入可永久留置等特點,達到眶緣結構良好的固定作用。同時在三維CT重建技術的指導下,眶骨骨摺診斷更準確,手術更精確,盲目操作風險性更小,手術時間縮短,大大提高瞭手術安全性。
목적탐토미형태판소형내고정정복술,재삼유CT중건기술적지도하,치료안광골절적림상효과。방법술전행횡단면、관상면이유CT소묘화삼유CT중건,근거삼유CT중건도상,채용미형태판소형내고정,대17례안광골절환자진행골절복위급안광골결손정복수술。결과17례술후복시급안구운동장애균소실,안구내함규정,시력무손해,차면부외관만의,무식입체륜곽음영혹계제상기형。CT소묘복사안광골절급결손처수복량호,연조직완전회납,태판무이위。술후수방3개월~1년,태판무배척반응、이위、감염급기타불량반응。결론미형태판소형내고정정복술,충분이용태판구유적량호생물상용성、견고강도대、가소형단우불역변형、체내식입가영구류치등특점,체도광연결구량호적고정작용。동시재삼유CT중건기술적지도하,광골골절진단경준학,수술경정학,맹목조작풍험성경소,수술시간축단,대대제고료수술안전성。
Objective To explore clinical effects of moulding internal fixation plasty with titanium mini-plate on orbital fracture under the guidance of 3D reconstruction technology. Methods Before the operation,cross-sectional,coronal 2D CT scan and 3D CT reconstruction were performed. Based on the 3D CT reconstruction image,moulding internal fixation plasty with titanium mini-plate mold was applied to 17 patients for repair of their orbital fracture and tissue defect. Results After the operation,diplopia and eyeball movement disorder of the 17 patients disappeared,enophthalmos was corrected,visual acuity was not damaged,and facial appearance was satisfactory;there was no implant outline shadows or ladder-shaped deformity. CT scan reexamination showed that the orbital fracture and soft tissue defect were repaired well,soft tissue was completely recovered,and the position of titanium plate did not change. The patients were followed up for three months to one year,during which,no rejection,infection and other adverse reactions caused by titanium plate were found. Conclusion Moulding internal fixation plasty with titanium mini-plate makes full use of the good biocompatibility and high strength of titanium plate,which can be molded but not easily deformed,and can be kept in human body permanently,achieving good fixation of the orbital margin structure. At the same time,under the guidance of 3D CT reconstruction technology,orbital fracture can be diagnosed more accurately and operation can be carried out more precisely,and blind operation risk becomes smaller and operation time can be shorten,which greatly improves the operation safety.
