目的评价腹腔镜下经胆总管术中置人鼻胆管在胆总管一期缝合术中的应用价值.方法回顾分析腹腔镜术中经胆总管放置鼻胆管一期缝合胆总管42例的临床资料.所有病例术前经选择,术中证实无胆管残余结石、狭窄及胆总管远端通畅,常规放置腹腔引流管.结果本组42例术中放置鼻胆管成功率100％,无一例发生胆漏.结论腹腔镜术中经胆总管逆行放置鼻胆管对预防一期缝合胆总管术后胆漏、胆管狭窄有广泛的应用价值,为胆总管一期缝合提供了安全保证.该方法操作简单、安全、可靠、有效.适合在多数医院推广应用.

Objective To evaluate the clinical value of nasobiliary tube placement during laparoscopic choledochotomy in primary suture of common bile duct.Methods A case-control study was performed on 42 patients undergoing this procedure.The patients who were chosen according to the inclusive criteria were confirmed as free of residual stones,stricture or obstruction of common bile duct.Abdominal drainage tubes were routinely placed.Results The nasobiliary tube was successfully placed on all of the pateints.No bile leakage was found.Conclusions Nasobiliary tube placement during laparoscopic choledochotomy is of great value in the prevention of bile leakage or bile duct stricture secondary to choledochotomy.This procedure is simple,safe,reliable,and effective.It is worth popularizing in most of the hospitals.
