目的:了解本地区急性细菌性结膜炎病原菌的种类分布及其对常用药物的敏感性,为临床诊断和治疗提供依据。方法:收集2010-01/2012-12河北邢台眼科医院诊治的急性细菌性结膜炎患者的结膜囊分泌物进行细菌学检查,并对致病菌种类及药敏试验结果进行分析。
<br> 结果:检标本186例中,培养阳性菌150例(80.6%),其中革兰氏阳性菌127例(84.7%),占前三位的分别是表皮葡萄球菌(28.0%),人葡萄球菌(10.0%),溶血葡萄球菌(4.0%)。革兰氏阴性菌23例(15.3%),前四位的分别是:大肠埃希菌、肺炎克雷伯菌、铜绿假单胞菌、阴沟肠杆菌,均为2.0%。革兰氏阳性菌除对万古霉素敏感率为100%外,对其他抗菌药物均有不同程度的耐药性。革兰氏阴性菌中敏感率最高的是头孢哌酮(87.0%)。
<br> 结论:表皮葡萄球菌等条件致病菌已成为本地区急性细菌性结膜炎的主要致病菌。无论革兰阳性菌还是革兰阴性菌对临床常用抗菌药物耐药性均较高。尽早获取结膜囊分泌物进行细菌培养和药敏试验,对于临床诊断及合理选择抗生素治疗意义重大。

<br> 結果:檢標本186例中,培養暘性菌150例(80.6%),其中革蘭氏暘性菌127例(84.7%),佔前三位的分彆是錶皮葡萄毬菌(28.0%),人葡萄毬菌(10.0%),溶血葡萄毬菌(4.0%)。革蘭氏陰性菌23例(15.3%),前四位的分彆是:大腸埃希菌、肺炎剋雷伯菌、銅綠假單胞菌、陰溝腸桿菌,均為2.0%。革蘭氏暘性菌除對萬古黴素敏感率為100%外,對其他抗菌藥物均有不同程度的耐藥性。革蘭氏陰性菌中敏感率最高的是頭孢哌酮(87.0%)。
<br> 結論:錶皮葡萄毬菌等條件緻病菌已成為本地區急性細菌性結膜炎的主要緻病菌。無論革蘭暘性菌還是革蘭陰性菌對臨床常用抗菌藥物耐藥性均較高。儘早穫取結膜囊分泌物進行細菌培養和藥敏試驗,對于臨床診斷及閤理選擇抗生素治療意義重大。
목적:료해본지구급성세균성결막염병원균적충류분포급기대상용약물적민감성,위림상진단화치료제공의거。방법:수집2010-01/2012-12하북형태안과의원진치적급성세균성결막염환자적결막낭분비물진행세균학검사,병대치병균충류급약민시험결과진행분석。
<br> 결과:검표본186례중,배양양성균150례(80.6%),기중혁란씨양성균127례(84.7%),점전삼위적분별시표피포도구균(28.0%),인포도구균(10.0%),용혈포도구균(4.0%)。혁란씨음성균23례(15.3%),전사위적분별시:대장애희균、폐염극뢰백균、동록가단포균、음구장간균,균위2.0%。혁란씨양성균제대만고매소민감솔위100%외,대기타항균약물균유불동정도적내약성。혁란씨음성균중민감솔최고적시두포고동(87.0%)。
<br> 결론:표피포도구균등조건치병균이성위본지구급성세균성결막염적주요치병균。무론혁란양성균환시혁란음성균대림상상용항균약물내약성균교고。진조획취결막낭분비물진행세균배양화약민시험,대우림상진단급합리선택항생소치료의의중대。
AIM: To investigate the distribution and drug resistance of the pathogenic bacteria in acute bacterial conjunctivitis in Xingtai, and to provide a reliable basis for clinical diagnosis and treatment.
<br> METHODS: Secretion specimens were taken from the patients with acute bacterial conjunctivitis in Xingtai Ophthalmic Hospital during January 2010 to December 2012 for bacterial culture. The pathogenic bacteria and drug sensitivity test were analyzed.
<br> RESULTS: Of the 186 specimens, 150 ( 80. 6%) were positive in culture, in which 127 ( 84. 7%) were gram-positive bacteria, the first three species were Staphylococcus epidermidis ( 42/127, 33. 1%), Human staphylococal ( 15/127, 11. 8%), Staphylococcus haemolyticus (6/127, 4. 7%). Twenty-three (15. 3%) were gram-negative bacteria, the first four species were Escherichia coli, Klebsiella pneumoniae, Pseudomonas aeruginosa and Enterobacter cloacae, the percentage of all resistance were 13. 0%. Gram-positive strains were 100% sensitive to vancomycin, and were resistance to other antibiotics with different degree. Gram negative bacteria had the highest rate of drug resistance to cefoperazone 87. 0%).
<br> CONCLUSION:Some opportunistic bacteria including Staphylococcus epidermidis have become the majority of pathogenic bacteria of acute bacterial conjunctivitis in this area. Both gram - positive bacteria and gram -negative bacteria were higher to drug resistance. Early bacterial culture combined with drug sensitivity test is grate significance for clinical diagnosis and reasonable clinical chose of antibiotics.
