目的:探讨大剂量沐舒坦在多重耐药细菌性肺炎中的作用。方法将64例仅对头孢哌酮/舒巴坦敏感多重耐药细菌性肺炎患者分为对照组和观察组,各32例,比较两组疗效、细菌转阴率、呼吸机时间、住院天数、死亡率及血气分析、炎性因子等指标。结果观察组疗效、细菌转阴率明显高于对照组( P<0.05或0.01),呼吸机时间、住院天数明显少于对照组(P<0.05),死亡率明显低于对照组(P<0.01)。观察组CRP、IL-1β、IL-6、IL-8、TNF-α、ANC较对照组显著下降(P<0.05或0.01),PaO2、PaO2/FiO2、IL-10、SOD较对照组显著升高(P<0.05或0.01)。结论大剂量沐舒坦对多重耐药细菌性肺炎疗效显著。

목적:탐토대제량목서탄재다중내약세균성폐염중적작용。방법장64례부대두포고동/서파탄민감다중내약세균성폐염환자분위대조조화관찰조,각32례,비교량조료효、세균전음솔、호흡궤시간、주원천수、사망솔급혈기분석、염성인자등지표。결과관찰조료효、세균전음솔명현고우대조조( P<0.05혹0.01),호흡궤시간、주원천수명현소우대조조(P<0.05),사망솔명현저우대조조(P<0.01)。관찰조CRP、IL-1β、IL-6、IL-8、TNF-α、ANC교대조조현저하강(P<0.05혹0.01),PaO2、PaO2/FiO2、IL-10、SOD교대조조현저승고(P<0.05혹0.01)。결론대제량목서탄대다중내약세균성폐염료효현저。
Objective To investigate the role of large dose ambroxol in the treatment of multi-drug resistant bacterial pneumonia. Methods 64 patients with multi-drug resistant bacterial pneumonia only sensitive to Cefopera-zone/sulbactam were randomly divided into two groups: the control group and the observation group. Their curative effect, bacterial clearance rate, ventilation time, hospital stay, mortality, blood gas analysis, and inflammatory factor were compared between the two groups. Results The curative effect and the bacteria clearance rate were significant-ly higher in the observation group than in the control group (P<0. 01). Mechanical ventilation time and duration of hospital stay were significantly less in the observation group than in the control group (P<0. 05). The mortality was significantly lower in the observation group than in the control group (P<0. 01). The levels of CRP, IL-1β, IL-6, IL-8, TNF-α, and ANC decreased more significantly in the observation group than in the control group (P<0. 05 or 0. 01). The levels of PaO2, PaO2/FiO2, IL-10, and SOD were significantly higher in the observation group than in the control group (P<0. 05 or 0. 01). Conclusion A large dose of ambroxol has significant therapeutic effect in the treatment of multi-drug resistant bacterial pneumonia.
