目的：探讨体外循环下慢性肺动脉血栓栓塞的手术治疗方法及围手术期的处理，以提高慢性肺动脉血栓栓塞的外科治疗水平。方法回顾性分析2006年1月至今9例慢性肺动脉血栓栓塞的患者，在低温停循环或间断深低温停循环下行肺动脉血栓取出及内膜剥脱术。结果全组9例无一例近期(3个月)死亡，术后患者临床症状均较术前明显减轻、活动能力明显提高、部分病例心功能完全恢复正常。结论体外循环下肺动脉血栓取出术和内膜剥脱术是慢性肺动脉血栓栓塞的有效治疗方法。
목적：탐토체외순배하만성폐동맥혈전전새적수술치료방법급위수술기적처리，이제고만성폐동맥혈전전새적외과치료수평。방법회고성분석2006년1월지금9례만성폐동맥혈전전새적환자，재저온정순배혹간단심저온정순배하행폐동맥혈전취출급내막박탈술。결과전조9례무일례근기(3개월)사망，술후환자림상증상균교술전명현감경、활동능력명현제고、부분병례심공능완전회복정상。결론체외순배하폐동맥혈전취출술화내막박탈술시만성폐동맥혈전전새적유효치료방법。
Objective To investigate the level of surgical treatment of pulmonary thromboendarterectomy and preioperative management of chronic pulmonary thromboembolism under extracorporeal circulation. Methods From January 2006 to September this year, 9 patients with chronic pulmonary thromboembolism underwent pulmonary thromboendarterectomy in assist of cardiopulmonary bypass. Results There was no short-term (3 months) death re-ported. The clinical symptoms, activity and cardiac function were improved in all patients. Conclusion Pulmonary thromboendarterectomy under extracorporeal circulation is an effective procedure in the treatment of chronic pulmonary thromboembolism.
