目的:应用彩色多普勒高频超声对原发性高血压患者股动脉结构的变化进行观察,探讨高频超声观察原发性高血压患者股动脉硬化情况的临床价值。方法选取2010年6月至2014年4月门诊及住院高血压患者100例(高血压组)为研究对象,应用彩色多普勒高频超声进行右侧股总动脉彩色多普勒超声检查,观察股总动脉内中膜回声及有无斑块形成,测量内-中膜厚度(IMT)及血流动力学情况,并与50例健康体检者(对照组)进行比较。结果高血压组右侧股总动脉内径[(7.57±1.10)mm]、IMT[(0.13±0.07)mm]、斑块检出率[64.00%(64/100)]及阻力指数(0.74±0.07)均明显高于对照组[(6.51±0.73)mm、(0.08±0.05)mm、28.00%(14/50)及0.67±0.05];收缩期峰值流速[(74.30±23.35)cm/s]及舒张期最低流速[(19.81±6.72)cm/s]均低于对照组[(80.39±10.94)、(26.85±5.28)cm/s],差异有统计学意义(P<0.05)。结论高血压与股总动脉粥样硬化密切相关,高频超声能迅速、准确地检测出原发性高血压患者股动脉硬化情况。

목적:응용채색다보륵고빈초성대원발성고혈압환자고동맥결구적변화진행관찰,탐토고빈초성관찰원발성고혈압환자고동맥경화정황적림상개치。방법선취2010년6월지2014년4월문진급주원고혈압환자100례(고혈압조)위연구대상,응용채색다보륵고빈초성진행우측고총동맥채색다보륵초성검사,관찰고총동맥내중막회성급유무반괴형성,측량내-중막후도(IMT)급혈류동역학정황,병여50례건강체검자(대조조)진행비교。결과고혈압조우측고총동맥내경[(7.57±1.10)mm]、IMT[(0.13±0.07)mm]、반괴검출솔[64.00%(64/100)]급조력지수(0.74±0.07)균명현고우대조조[(6.51±0.73)mm、(0.08±0.05)mm、28.00%(14/50)급0.67±0.05];수축기봉치류속[(74.30±23.35)cm/s]급서장기최저류속[(19.81±6.72)cm/s]균저우대조조[(80.39±10.94)、(26.85±5.28)cm/s],차이유통계학의의(P<0.05)。결론고혈압여고총동맥죽양경화밀절상관,고빈초성능신속、준학지검측출원발성고혈압환자고동맥경화정황。
Objective To observe the changes of femoral artery structure by applying high frequency color Doppler ultra-sonography for patients with primary hypertension,and explore the clinical value of high-frequency ultrasonogarphy for femoral artery sclerosis in patients with primary hypertension. Methods One hundred patients with primary hypertension,who were treated in the outpatient service and hospitalized in the hospital from June 2010 to April 2014,were selected as objects(hyperten-sion group),and the high-frequency color Doppler ultrasonography was applied in right common femoral artery to observe the inti ma-media echo and plaque,measure intima-media thickness(IMT) and haemodynamics,which were compared with other 50 healthy controls(control group). Results The inner diameter of right common femoral artery,IMT,detection rate of plaque and resistance index in the patients with primary hypertension[(7.57±1.10)mm,(0.13±0.07)mm,64.00%(64/100) and 0.74±0.07 respectively] were obviously higher than those in the control group[(6.51±0.73)mm,(0.08±0.05)mm,28.00%(14/50) and 0.67± 0.05 respectively],the peak systolic velocity and lowest diastolic velocity in the hypertension group[(74.30±23.35)cm/s,(19.81± 6.72)cm/s] were both inferior to the control group [(80.39±10.94)cm,(26.85±5.28)cm/s],and the differences had statistical signif icant(P<0.05). Conclusion Hypertension is closely correlated with common femoral artery atherosclerosis,and high-frequency color Doppler ultrasonography can swiftly and accurately check femoral artery atherosclerosis in patients with hypertension.
