目的：探讨微创髋关节置换术治疗老年股骨颈骨折临床疗效。方法选择本院2009年3月至2012年3月髋关节置换术手术患者100例，随机分为观察组和对照组，观察组行微创髋关节置换术，对照组行常规人工髋关节置换术。记录两组患者手术时间、手术出血量及切口长度，对两组患者术后随访进行Harris评分并进行对比。结果观察组切口长度、手术出血量分别与对照组比较，差异有统计学意义（P<0.05），手术时间较对照组无差别（P>0.05）；两组患者术后评分比较，3个月前差异有统计学意义（P<0.05），3个月后差异无统计学意义（P>0.05）。结论微创髋关节置换术治疗老年股骨颈骨折具有手术出血少，创伤小，术后早期功能恢复快，并发症少，临床疗效好，值得临床推广。
목적：탐토미창관관절치환술치료노년고골경골절림상료효。방법선택본원2009년3월지2012년3월관관절치환술수술환자100례，수궤분위관찰조화대조조，관찰조행미창관관절치환술，대조조행상규인공관관절치환술。기록량조환자수술시간、수술출혈량급절구장도，대량조환자술후수방진행Harris평분병진행대비。결과관찰조절구장도、수술출혈량분별여대조조비교，차이유통계학의의（P<0.05），수술시간교대조조무차별（P>0.05）；량조환자술후평분비교，3개월전차이유통계학의의（P<0.05），3개월후차이무통계학의의（P>0.05）。결론미창관관절치환술치료노년고골경골절구유수술출혈소，창상소，술후조기공능회복쾌，병발증소，림상료효호，치득림상추엄。
Objective?To?explore?the?minimally?invasive?total?hip?arthroplasty?for?femoral?neck?fractures.?Methods?100?patients?in?our?hospital?from?2009?March?to?2012?March?total?hip?arthroplasty?operation?patients,?randomly?divided?into?the?observation?group?and?the?control?group,?the?observation?group?underwent?minimally?invasive?hip?replacement,?the?control?group?underwent?conventional?total?hip?arthroplasty.?Record?two?groups?of?patients?with?operation?time,?bleeding?volume?and?length?of?incision?operation,?Harris?score?of?follow-up?after?operation?in?the?two?groups?were?compared.?Results?In?the?observation group, incision length, operation bleeding volume were compared with the control group, the difference was statistically signiifcant (P<0.05),?operation time compared with the control group no difference(P>0.05);two groups of patients with postoperative score comparison, statistically signiifcant differences in 3 months ago(P<0.05), 3 months after the difference was not statistically signiifcant (P>0.05).?Conclusion?Minimally?invasive?hip?replacement?in?the?treatment?of?senile?femoral?neck?with?operation?less?bleeding,?less?trauma?fracture,?early?postoperative?functional?recovery,?less?complications,?clinical?curative?effect?is?good,?is?worth?the?clinical?promotion.
