目的:探讨心力衰竭患者心电图QRS波宽度与血浆脑利钠肽(BNP)的相关性,评价其判断心力衰竭患者的临床心功能的预测价值。方法根据心力衰竭诊断标准(Framingham标准),入选152例患者,明确诊断为心力衰竭。152例患者分别测定心电图的QRS的时限、血浆BNP、左室舒张末径(LVDD)及左室射血分数(EF),并根据QRS的时限分为正常组及异常组(增宽≥120 ms),数据进行统计分析。结果 QRS时限正常组中脑利钠肽与QRS的时限异常组中脑利钠肽两两相比较有显著性差异。结论宽QRS(时限≥120 ms)心力衰竭患者的心力衰竭程度明显高于QRS正常者,用于评价心力衰竭患者病情有一定的临床意义。

목적:탐토심력쇠갈환자심전도QRS파관도여혈장뇌리납태(BNP)적상관성,평개기판단심력쇠갈환자적림상심공능적예측개치。방법근거심력쇠갈진단표준(Framingham표준),입선152례환자,명학진단위심력쇠갈。152례환자분별측정심전도적QRS적시한、혈장BNP、좌실서장말경(LVDD)급좌실사혈분수(EF),병근거QRS적시한분위정상조급이상조(증관≥120 ms),수거진행통계분석。결과 QRS시한정상조중뇌리납태여QRS적시한이상조중뇌리납태량량상비교유현저성차이。결론관QRS(시한≥120 ms)심력쇠갈환자적심력쇠갈정도명현고우QRS정상자,용우평개심력쇠갈환자병정유일정적림상의의。
Objective To explore the relationship between electrocardiogram QRS duration and plasma B-type natriuretic peptide (BNP) in patients with heart failure. To evaluate the predictive value of QRS duration on clinical heart function in patients with heart failure. Methods According to the diagnostic standard of heart failure (Framingham standard), electrocardiogram QRS duration, plasma B-type natriuretic peptide (BNP), left ventricular end diastolic diameter (LVDD), and left ventricular ejection fraction (EF) were measured in 152 patients with heart failure. Then, according to QRS duration, 152 patients with heart failure were divided into normal group and abnormal group (QRS duration≥120 ms). Finally, we conducted statistical analysis of data. Results BNP was signiifcantly different between normal group and abnormal group. Conclusion The heart function classiifcation of wide QRS (QRS duration≥120 ms) is signiifcantly higher than that of normal QRS in patients with heart failure. QRS duration has the certain clinical signiifcance in evaluation of heart failure patients.
