目的:探讨妊娠糖尿病(GDM)患者血清同型半胱氨酸(Hcy)、胱抑素C(CysC)水平的变化及其与胰岛功能的关系。方法选择2012年1月至2012年12月门诊确诊的240例GDM患者作为观察组,100例同期健康孕妇作为对照组。通过对血Hcy、CysC、空腹血糖(FBG)、空腹胰岛素(FINS)的测定,比较两组患者血Hcy、CysC、FBG、FINS、HOMA-IR、HOMA-β水平,并分析血Hcy、CysC与HOMA-IR、HOMA-β的相关性。结果观察组Hcy、CysC、FBG、FINS、HOMA-IR明显高于对照组,两组间HOMA-β无明显差别;Hcy、CysC与HOMA-IR正相关,与HOMA-β无明显相关性。结论GDM患者HOMA-IR、FINS、Hcy、CysC明显升高,Hcy、CysC与HOMA-IR正相关。提示GDM患者存在胰岛素抵抗,血Hcy、CysC水平的测定可作为GDM患者的监测指标。

목적:탐토임신당뇨병(GDM)환자혈청동형반광안산(Hcy)、광억소C(CysC)수평적변화급기여이도공능적관계。방법선택2012년1월지2012년12월문진학진적240례GDM환자작위관찰조,100례동기건강잉부작위대조조。통과대혈Hcy、CysC、공복혈당(FBG)、공복이도소(FINS)적측정,비교량조환자혈Hcy、CysC、FBG、FINS、HOMA-IR、HOMA-β수평,병분석혈Hcy、CysC여HOMA-IR、HOMA-β적상관성。결과관찰조Hcy、CysC、FBG、FINS、HOMA-IR명현고우대조조,량조간HOMA-β무명현차별;Hcy、CysC여HOMA-IR정상관,여HOMA-β무명현상관성。결론GDM환자HOMA-IR、FINS、Hcy、CysC명현승고,Hcy、CysC여HOMA-IR정상관。제시GDM환자존재이도소저항,혈Hcy、CysC수평적측정가작위GDM환자적감측지표。
Objective?To explore the changes of homocysteine(Hcy)and cystatin C(CysC) level in the serum of gestational diabetes mellitus patients(GDM), and?the?relationship?between?the?changes?and?islet?function.?Methods?Selecting?240?cases?of?GDM?diagnosed?from?outpatient?from?January?to?December?in?2012?as?a?observation?group,?and?selecting?100?cases?of?healthy?pregnant?women?over?the?same?period?as?a?control?group.?To?determine?Hcy,?CysC,?fasting?blood glucose(FBG)and fasting insulin(FINS)in the blood, and comparing the levels of Hcy, CysC, FBG, FINS, HOMA-IR and HOMA-βin the blood between these two groups. Then analyzing the correlation between Hcy, CysC and HOMA-IR, HOMA-β. Results?The?levels?of?Hcy,?CysC,?FBG,?FINS?and HOMA-IR in the observation group are signiifcantly higher than the ones in the control group. There are no signiifcant differences of HOMA-β’s level between the two groups. The levels of Hcyand CysC are positively correlated with HOMA-IR’s level, but there is no signiifcant correlation with HOMA-β’s level. Conclusions?The levels of Hcy, CysC, FINS and HOMA-IR of GDM patients were signiifcantly increased, and the levels of Hcy and CysC are positively correlated with HOMA-IR’s level. To prompt the GDM patients have insulin resistance. To prompt the determination of the levels of Hcy and CysC in the blood?can?be?the??monitoring?indicators?of?GDM?patients.
