目的：探讨临床药师对临床药学监护的方法。方法通过参与1例肺栓塞合并慢性阻塞性肺疾病急性加重患者的全程药学监护，从抗凝和抗感染两方面协助医师制订治疗方案。结果与结论患者用药方法得以正确指示。临床药师协助医师制定个体化的治疗方案，有利于提高临床治疗水平，促进合理用药。
目的：探討臨床藥師對臨床藥學鑑護的方法。方法通過參與1例肺栓塞閤併慢性阻塞性肺疾病急性加重患者的全程藥學鑑護，從抗凝和抗感染兩方麵協助醫師製訂治療方案。結果與結論患者用藥方法得以正確指示。臨床藥師協助醫師製定箇體化的治療方案，有利于提高臨床治療水平，促進閤理用藥。
목적：탐토림상약사대림상약학감호적방법。방법통과삼여1례폐전새합병만성조새성폐질병급성가중환자적전정약학감호，종항응화항감염량방면협조의사제정치료방안。결과여결론환자용약방법득이정학지시。림상약사협조의사제정개체화적치료방안，유리우제고림상치료수평，촉진합리용약。
Objective To discuss the pharmaceutical care mode of clin-ical pharmacists.Methods The pharmaceutical care that clinical phar-macists participated in for a patient with pulmonary embolism complicated with acute exacerbation of chronic obstructive pulmonary disease was ana-lyzed from the perspective of anticoagulation and anti -infection retro-spectively.Results and Conclusion With the pharmaceutical care of clinical pharmacists , the patient's condition was improved.Clinical phar-macists'participation in clinical treatment could be helpful to develop in-dividualized treatment , improve the level of clinical treatment and pro-mote rational drug use.
