目的:研究结直肠癌中K-ras基因突变率及突变类型,探讨结直肠癌K-ras基因野生型及突变型与患者临床参数的关系;通过基因测序,指导临床靶向药物用药。方法随机选取哈尔滨医科大学附属第三医院结直肠外科2008年9月至2010年3月收治的100例结直肠癌患者组织标本通过手术室取材、组织标本DNA提取、逆转录PCR、凝胶电泳、胶回收、直接测序等技术筛选K-ras基因突变标本进行统计分析,从中得出突变率及突变类型。结果共筛选出了34例(34%)K-ras基因突变标本,其基因突变均发生在第一外显子的第12及第13密码子。结果表明:第一外显子的第12密码子GGT突变为GAT或GTT;第一外显子第13密码子由GGC突变为GGT。其中并未发现第二外显子第59及第61位发生突变,及第一外显子12位13位同时突变。结论 K-ras基因结直肠癌中约有34%发生突变。在K-ras基因突变类型与患者的病期、病理类型、肿瘤的浸润深度、发病部位等因素无关。

목적:연구결직장암중K-ras기인돌변솔급돌변류형,탐토결직장암K-ras기인야생형급돌변형여환자림상삼수적관계;통과기인측서,지도림상파향약물용약。방법수궤선취합이빈의과대학부속제삼의원결직장외과2008년9월지2010년3월수치적100례결직장암환자조직표본통과수술실취재、조직표본DNA제취、역전록PCR、응효전영、효회수、직접측서등기술사선K-ras기인돌변표본진행통계분석,종중득출돌변솔급돌변류형。결과공사선출료34례(34%)K-ras기인돌변표본,기기인돌변균발생재제일외현자적제12급제13밀마자。결과표명:제일외현자적제12밀마자GGT돌변위GAT혹GTT;제일외현자제13밀마자유GGC돌변위GGT。기중병미발현제이외현자제59급제61위발생돌변,급제일외현자12위13위동시돌변。결론 K-ras기인결직장암중약유34%발생돌변。재K-ras기인돌변류형여환자적병기、병리류형、종류적침윤심도、발병부위등인소무관。
Objective To study the K-ras gene mutation rate and its types of mutation in color-ectal carcinoma.To correlate wild type and mutation type of K-ras gene with clinicopathologic parameters.Through gene sequencing it to guide clinical application of targeted drugs.Meth-ods One hundred samples of colorectal cancer which tissues were collected from the Third Af-filiated Hospital of Harbin Medical University from September 2008 to March 2010.Tissue DNA extraction, real-time quantitative polymerase chain ( PCR) , gel electrophoresis, gel recy-cling, direct sequencing screening of k-ras gene mutation specimens, and statistic analysis were adopted to find the mutation rate and mutation characteristics.Results 34 k-ras gene mutation cases were screened out (34%) in specimens, which all occurred in the first exon of the 12th and 13th.The results showed that the first exon of codon 12th GGT mutated into GAT or GTT, and 13th the first exon of codon GGC mutated into GGT.However, no mutations in the second exon of the second of the 59th or in the third exon of the 61th were found in these specimens. Conclusion About 34%of k-ras gene mutates in colorectal carcinoma patients.The types of k-ras gene mutation are not related to clinicopathologic parameters.
